| Objective: The study was to investigate changes of economic operation in township hospitals of Shanxi caused by the implementation of essential medicines and take a research of the equipment of essential medicines and their affordability of township hospitals of Shanxi then discuss countermeasures.Methods: Take analysis of indicators including medical service providing amount,the balance of payment structure and average outpatient costs before and after the implementation of essential medicines system of a township hospital.DEA-malmquist was used to analysis the data of township hospitals in Shanxi,and the efficiency was evaluated.Take a research of the equipment of essential medicines and their affordability of township hospitals of Shanxi based on the standard methodology which was developed by the World Health Organization and Health Action International.Results : The overall outpatient number of township hospitals of Shanxi showed increasing trend while the hospital admission number find the opposite way. In 7 years Shanxi’s total factor productivity have declined 1.2% per year. Total factor productivity of5 cities of Shanxi have increased while the other 6 cites of Shanxi have declined.The equipment of essential medicines of township hospitals of Shanxi was below average and vary from classification in some rules. Population density was positive correlated with the equipment of essential medicines. The equipment of essential medicines of central township hospitals of Shanxi was highly above the equipment of essential medicines of rural township hospitals of Shanxi.The equipment of chemical essential medicines was above the equipment of essential Chinese traditional medicines of township hospitals of Shanxi. The affordability of essential medicines was above average. The affordability of essential Chinese traditional medicines was below the affordability of chemical essential medicines.Conclusion : The implementation of basic drug system has achieved good results in the control of drug price. But it has great impact to township hospital economic operation which reduced its profitability to some extent. Productivity of Shanxi township hospitals needs to be improved. The equipment of essential medicines of township hospitals of Shanxi needs to be improved. The affordability of essential Chinese traditional medicines needs to be improved. It is recommended to improve economic compensation mechanism,ensure the accommodate of essential medicines in township hospitals and improve the corresponding support policies and increase policy advocacy to enhance the service capabilities of township hospitals. |
