Objective When the body is subject to traffic accidents, falls and other injuries caused by acute spinal cord injury, if not timely and effective treatment, often resulting in varying degrees of paraplegia. In recent years, it is found that blocking the interleukin-6(IL6) in the acute phase of spinal cord injury can interfere with the initial inflammatory response, so as to protect the surviving spinal cord tissue and improve the prognosis. So in this study, We combined monoclonal antibody against interleukin-6 and bone marrow stromal stem cells, then detecd by hind limb kinematics score, muscle strength score and TUNNEL cell apoptosis detection method, to observe the the efficacy of combined therapy and the improvement of cell apoptosis.Method Totally 30 healthy Sprague-Dawley(SD) rats, weighing 250-300 g, were randomly divided into 5 groups(n=6 in each group): sham-operated group, acute spinal cord injury group, bone marrow stromal cells transplantation group, monoclonal antibody against interleukin-6 transplantation group, Combination therapy group of monoclonal antibody against-6 and bone marrow stromal cells(hereinafter referred to as combination therapy group). In 2 days after the surgery, each group was injected liquid of 2.5ul in injury site,and the sham operation group and the acute spinal cord injury group were injected with the same amount of normal saline. we observed the kinematics of the of 1d before treatment and 1,3,5,7,14,28 d after treatment. We collect the spinal cord specimens of 28 d after the operation for apoptotic cell detection.Result Kinematic results:The lower extremity kinematics score(BBB) and modified T score of sham-operated group were all perfect scores. After surgery, except the Sham-operated group,the other group lost the ability to lower limb. After 3 days of treatment, lower limb activity of the treatment group began to recover. Compared with the same period in the acute spinal cord injury group, the significant increases of lower limb activity score were found in the treatment groups(P<0.05). The lower limb activity score of combination therapy group was higher than the other treatment groups(P<0.05).Apoptotic cell detection results:apoptosis was significantly increased of acute spinal cord injury group compared to sham-operated group(P<0.05). Apoptosis rate of the treatment group was markedly lower than that of acute spinal cord injury group(P<0.05). Apoptosis rate of combination therapy group was significantly lower than that of the other groups(P<0.05).Conclusion In rats,both monoclonal antibody against interleukin-6 and bone marrow stromal stem cells can improve the neurological recovery of acute spinal cord injury, and reduce the apoptosis of the injured site. The combined treatment effect is better.
