[aims]To acclimate the prevalence of minor invasion and localization in surgery, in the precondition of ensure affecting a radical cure in high anal fistula, the purpose is to remain the integrality and continuity of anorectum ring maximally, and at the same time reduce the injury to the crissal tissues to retain the normal function of anus. [methods]30 cases of high anal fistula with clear inner port and without taboos of operation were divided into 2 groups. In therapeutic group, low incision and high loose thread-drawing drainage method of operation was used, and anorectum ring was preserved completely. In control group, traditional low incision and high thread-drawing drainage method of operation was used, and chronic incisions after operation. General curative effect, anal deformation, anal resting pressure, anal maximal systolic pressure, postoperation complications (leakage of gas and liquid, key-hole deformation) and recurrence of anal fistula were observed in the two groups after operation. [results]There are no significant differences of general curative effect and recurrence of anal fistula. There are significant differences of anal deformation, anal resting pressure, anal maximal systolic pressure, postoperation complications (leakage of gas and liquid, key-hole deformation). [conclusions]The low incision and high loose thread-drawing drainage method of operation can protect the function of anarectum ring, and this method is doable to affect a radical cure in high anal fistula.
