| Objective: To research the relationship by X-ray cephalometric measuring in adolescents who were after growth peak with Angle class? division 2 and non-extraction by self-ligating brackets,compared the incisive inclination before and after the patients treated.Analysis the incisive compensatory and the change in incisive convexity in order to provide the clinical treatment planning and specific diagnosis.Methods: Total 112 Angle class? division 2 adolescent patients after the peak of growth,between 2008 and 2013 to Stomatology Hospital of Shen Yang Orthodontics department for treatment.This paper randomly selected 15 adolescent patients who had been treated by non-extraction were over.There were 4 males,11 females,aged between 12-17 years old,the average age is 14.3 years old,average course of treatment is 23 months.They have the same inclusion criteria: permanent dentition period,based on the cervical vertebral bone quantitative stage after Cvs4,with complete dentition skeletal ? or ?,vertical facial types were in average angle,typical distal molars and canines relationship,incisive atresia relationship,andII or III of deep overbite,Bolton index coordination,upper and lower arch with mild or moderate crowding;periodontal health,had no temporomandibular joint disease,no bad oral habits and had no history of orthodontic treatment.The treatment process of patients use 3M smartclip self-ligating brackets,and use flat guide plate for bite opening.The relationship between the position of the upper jaw and the mandible was adjusted by the Class ? traction.Before and after the orthodontic treatment,the patients used the same American Dentsply ORTHORLIX,S-type panoramic X-ray machine for inspection,under the strict head location taken the cephalometric X-ray,and analysis the X-ray measurement.Incisor inclination on measurement using the method of Downs,reference Wits analysis and Tweed analysis,selected 20 items for the data which represented soft tissue and bone tissue.Focus on the analysis the differences between making the comparison to the normal occlusion group and between before and after treatments.All the selected data were analyzed by SPSS19.0statistical software,when P <0.05 we considered had stasitic signification.Results: After active treatment,all the patients had been better overjet and overbite than those in pre-treatment,and a good facial profile and occlusion were achieved.After treatment,the angles of incisor inclination,such as U1-SN(°)?U1-NA(°)?L1-NB(°)?L1-MP(°)?U1-NA(mm)?L1-NB(mm)?U1-Npog(mm)?L1-Npog(mm)were increased(P<0.01),these data had significant differences,the incisor inclination became more libially.Meanwhile,U1-L1(°)decreased(P<0.01),Wits(mm)increased(P<0.01),LL-E line(mm)increased(P<0.05),LL-B'-Pog'(°)increased(P<0.05);SNB(°)increased(P<0.05),ANB(°)decreased(P<0.01)these data had statistically significantion by prompting the mandibular of patients had been grown.The other data was no statistically significant.Conclusion:(1)Angle?division 2 patients who were after the peak of growth,the growth of mandible is limited.Treatment of maxillary anterior teeth with lingual inclination,the mandibule was released after the improvement of the relationship between the locking of the upper and lower anterior teeth.According to this study,the patients contrasted before and after treatment can draw a conclusion that incisiveinclination can change wildly,the compensation of lower incisor flared more than normal.(2)The self-ligating brackets can quickly correct lingual inclination of anterior teeth,can quickly solve the crowded teeth.Because of its low friction,high treatment efficiency,with the participation of the soft tissues can be better controlled on the lip inclination of anterior teeth,the profile is also improved.Clinicians may select non-extraction for the adolescent patients for reduce the difficulty of treatment.The occlusion can be corrected with orthodontic treatment,the facial profile also can be treated better. |
