Objective: Using field investigation to obtain the empirical data,to calculate disease economic burden of patients with lung cancer in Nanchang,and analyze the effect of current medical insurance policies on reducing the economic burden of lung cancer,to give advice of controlling or reducing the economic burden of it.To promote the reasonable application and prior input of regional health resources.Methods: Using the technique of field investigation and questionnaires,to obtain Lung cancer patients on Sep.1,2014 to Jan.1,2015 in Nanchang.According to the informed consent principle,interviewed the patients and caregivers to collect data.Using Epi Data and SPSS17.0 to create the database,and to do the statistical analysis.Descripted the demographic data of the patients,calculated disease economic burden of patients with lung cancer,used the Kruskal-Wallis nonparametric test to explore the influence factors of direct economic burden of lung cancer patients and also took Kruskal-Wallis nonparametric test to analyze the influence of different medical insurance policy on the economic burden.Results: 203 patients included in the research,146 cases are male patients,57 patients are female patients,the sex ratio is 2.56:1,and the average age of patients is 59.76.91.6% patients are married or cohabiting.Patients generally had low education.Patients buy the medical insurance in different forms;the majority is New Rural Co-operative Medical System.Lung cancer patients had the heavy economic burden.The per capita economic burden for patients was 126 075.53 yuan,the direct economic burden was 99 531.14 yuan and the proportion was 78.95%,the indirect economic burden was 26 544.09 yuan,accounting for 21.05%.hospitalization expenses is the main source of economic burden of patients and direct medical costs account for larger proportion in the direct economic burden.The high catastrophic health expenditure led to the high poverty rate caused by diseases.The incidence rate of catastrophic health expenditure was 82.3%,most of the families suffered the high risk of poverty caused by diseases.After the compensation of primary medical insurance,the incidence rate was 47.3%,so the current primary medical insurance can reduce the risk of poverty due to illness,to a certain extent,and reduce the proportion of catastrophic health expenditure's family.The primary medical insurance policy can reduce the economic burden of patients in some degree,but the actual reimbursement was not satisfied.The proportion of reimbursement of direct medical expenses for Per capita outpatient was 13.78%,while the proportion was 57.02% for direct hospitalization medical expenses.The hospitalization expenses existed significant differences between different medical insurances(?2=32.410,p=0.000<0.05).Free medical care and the medical insurance for urban workers in a large extent alleviated the patients' economic burden;however,Urban Residents Basic Health Insurance(URBMI)and new rural cooperative medical system(NCMS)had a little influence.Suggestion: Controlling the hospitalization days properly,to cut down hospitalization expenses,and reduce lung cancer patients' economic burden.All kinds of ways should be taken to help lung cancer patients to reduce the incidence of catastrophic health expenditure.Deepen the health care system reform,reduce drug costs,value the doctors' technology properly,and regulate the behaviors of inspection diagnosis and treatment.Explore hospital-community-family care mode actively,to provide multi-level and seamless care services.To promote the coordinated development of medicine insurance policy,improve the reimbursement level.Raise people's income to enhance the ability of lung cancer patients and families to pay.
