Application Of 3.0T MRI In Pre-operative Staging Of Endometrial Carcinoma

Objective To investigate the application value of plain scan and contrast-enhanced 3.0 Tesla magnetic resonance imaging(MRI)unite diffusion-weighted imaging(DWI)in preoperative staging of endometrial carcinoma(EC).Methods Retrospective analysis was adopted to analyse the data of 202 patients who were suffered endometrial carcinoma involvement from January 2012 to August 2014 in the First Affiliated Hospital of Zheng Zhou University. MRI examination adopts Siemens Skyra 3.0 Tesla MRI scanners and body phased front circle.Regular fat saturation scanning T2 WI,line transverse T1 WI and DWI, can coronary fat saturation T2 WI and sagittal position T2 WI scanning.Dynamic contrast-enhanced(DCE) using the isotropic spatial resolution 3D rf interference phase GRE sequence lines cross sectional fat saturation scanning, complete sagittal,delay after transverse and coronal scanning, scanning parameters with scan T1 WI.MRI staging of endometrial carcinoma based on the new FIGO 2009,T2 WI, DWI and DCE. 202 cases of endometrial carcinomas were compared with surgical pathologic staging,which was considered as the gold standard.Some clinical dates were calculated of MRI staging including the specificity,sensitivity,negative predictive,positive predictive,and Youden index.Results 1 ? 202 cases of endometrial carcinomas patients with surgical pathology confirmed 131 cases of staging IA,28 cases of staging IB, 15 cases of staging II,3 cases of staging IIIA,16 cases of staging IIIB,4 cases of staging IIIC1,1 cases of staging IIIC2,1 cases of staging IVA,3 cases of staging IVB. 2?The preoperation staging result of plain scan and contrast-enhanced 3.0Tesla MRI unite diffusion-weighted imaging was 140 cases of staging I,20 cases of staging IB, 18 cases of staging II,5 cases of staging IIIA,13 cases of staging IIIB,2 cases of staging IIIC1,1 cases of staging IIIC2,1 cases of staging IVA,2 cases of staging IVB.Nine patients with stage IB misdiagnosed as IA; 3 patients with stage IIIB misdiagnosed as II,and 1 case was misdiagnosed as stage IIIA; 1 case IIIC1 misdiagnosed as IB and 1 case was misdiagnosed as stage IIIA; 1 case of stage IVB misdiagnosed as IIIB. 3?As to endometrial carcinoma, the 3.0T MRI diagnostic coincidence rate with pathological staging was 92.1%( 186/202).The specificity,sensitivity,negative predictive,positive predictive,and Youden index of staging I were 76.7%?94.3%?78.6%?93.8%?0.71,staging II were 98.4%?100.0%?100.0%?83.3%?0.98,staging III were 98.3%?75.0%?96.7%?85.7%?0.73 and staging IV were 100.0%?75.0%?99.5%?100.0%?0.75.Conclusions In endometrial carcinoma, 3.0T MRI has superior accuracy in preoperative staging,which can be used for endometrial cancer patients preoperative routine inspection, so as to provide objective evidence for the clinical decision.
