ObjectiveThrough the retrospective analysis of the clinical data of treatment of patent ductus arteriosus by minimally invasive occlusion and by transcatheter intervention.comparison and analysis the superiority and the difference of two treatments, explore the individualized treatment of PDA, help patients to reference to choose the appropriate treatment. Methods1.A retrospective analysis of the First Affiliated Hospital of Zhengzhou University from September 2013 to September 2015, has been ruled out other cardiovascular malformations 79 patients who underwent transthoracic minimally invasive transcatheter therapy PDA patients and 45 patients underwent transcatheter interventional occlusion therapy the clinical data.Comparative analysis of two cases:(1)General information: such as men and women in both groups constitute ratio, average age, average weight, PDA diameter, blood oxygen saturation, etc;(2)The operation time of two treatments, postoperative hospital stay, total hospital stay, total costs;(3)Success rate of surgery, Instant complete closure rate, long-term complete closure rate, incidence of major complications;(4)the pulmonary artery pressure( or aortic pressure)before and after operation,its pressure differences.(5)the Tei index of Left ventricular;(6)Success rate in patients with pulmonary hypertension by two kinds of operations;(7)Comparison of the incidence of surgical complications. Results1.No significant difference between the two groups of patients with general information(P>0.05).2.Differences on the operation time,P<0.05,There are statistical significance;Differences on Hospital stay after surgery,P>0.05,No statistical significance;Differences on Total hospital charges,P<0.05,There are statistical significance.3.Differences on Success rate of surgery, P < 0.05, There are statistical significance;Differences on Instant complete closure rate,P<0.05,There are statistical significance;Differences on Long-term complete closure rate,P<0.05,There are statistical significance;4.Differences on the pulmonary artery pressure( or aortic pressure)before and after operation,P>0.05,No statistical significance;and its pressure differences,too.5.Differences on the Tei index of Left ventricular,P>0.05,No statistical significance;6.Differences on the incidence of surgical complications,P<0.05,There are statistical significance;7.Differences on Success rate in patients with pulmonary hypertension by two kinds of operations,P<0.05,There are statistical significance; Conclusions(1)For patients with PDA in preterm children, newborn, young, low birth weight, peripheral vascular conditions have certain limitations, transthoracic minimally invasive transcatheter therapy is the best choice, with other surgical incomparable advantages.(2)For non-pulmonary hypertension PDA, no significant difference on success rate,cardiac function index, postoperative complications between minimally invasive occlusion and transcatheter interventional, but the minimally invasive occlusion spend less, save more medical resources.(3)For PDA merge pulmonary hypertension,high success rate of surgery, less postoperative complications,minimally invasive occlusion has more advantages than transcatheter interventional.All in all, minimally invasive occlusion is more simple, safer, more reliable, wider surgical indications, and lower cost.It has huge development potential and broad application prospects.
