| Purpose The purpose of this article is to evaluate the diagnostic value of magnetic resonance imaging(MRI)and clinical examination on rupture of the supraspinatus tendon.Method Since April 2014--December 2015,MRI,Jobe test,Neer test and three clinical examinations of descend arm test were first conducted on 38 cases of suspected rupture of the supraspinatus tendon,and Arthroscopic shoulder surgery was conducted later.The examination result of arthroscopy is taken as a golden standard to diagnose rupture of the supraspinatus tendon.Comparison is conducted on MRI and result of clinical examination.Men 24 cases,14 cases,women aged 40 to 71 years old,patients with clinical manifestations are mainly: of shoulder joint pain and restricted movement,lift on outreach,cannot,in which 12 cases had obvious history of trauma,the remaining cases were chronic strain.With arthroscopic shoulder surgery as a diagnostic test results of the supraspinatus tendon tear,"gold standard" of nuclear magnetic resonance imaging(MRI) to check and compare analysis of the results of clinical examination.Result According to the arthroscopy surgery examination,there are 26 cases of rupture of the supraspinatus tendon,and 12 normal cases.MRI examination shows that there are 25 cases of rupture of the supraspinatus tendon and 13 normal cases.Jobe test,Neer test and three clinical examinations of descend arm test have diagnosed 16 cases of rupture of the supraspinatus tendon and 22 normal cases.MRI diagnosis result:25 cases of true positive,11 cases of true negative,1 case of false positive and 1 case of false negative.The degree of accuracy,sensitivity and specificity is 94.7%,96.1% and 91.6% respectively.The medical result of clinical examination: 14 cases of true positive,10 cases of true negative,and 2 cases of false positive and 12 cases of false negative.The degree of accuracy,sensitivity and specificity is 68.4%,53.8%,and 83.3% respectively.The accuracy of MRI in diagnosing rupture of the supraspinatus tendon is significantly higher than clinical examination(P<0.05)).Conclusion The false negative rate of rupture of the supraspinatus tendon in clinical examination is high;the accuracy of MRI on diagnosing rupture of the supraspinatus tendon id high,which is the effective method to diagnose rupture of the supraspinatus tendon.But since the 1990 s with the nuclear magnetic resonance imaging(MRI)diagnosis technology unceasing enhancement,magnetic resonance imaging(MRI)has been widely applied to the clinical diagnosis of rotator cuff injury.Magnetic resonance imaging(MRI)examination in diagnosis the supraspinatus tendon imaging examination method of the most accurate,noninvasive,no radiation,show the sequence,more comprehensive,more parameters,the advantages of plane,preoperative magnetic resonance imaging(MRI)examination may also found other lesions in the shoulder joint and other components of rotator cuff injury,for diagnosis and arthroscopic surgery provides very powerful evidence. |
