Background Nowadays,Diabetes mellitus,DM,is a very common disease,which is also a serious threat to human health worldwide.In recent years,the worldwide diabetes prevalence and incidence rate are increasing rapidly.For China,because of the rapid economic development,westernized lifestyle and increasing rates of aging population and obesity,the prevalence of DM also shows a rapid growth trend.As numerous studies show,the harm from DM to human health is far beyond the hyperglycemia itself,various acute and chronic complications caused by DM have a serious impact on the lives and health quality of patients.Compared to non-DM patients,the DM patients are more susceptible to various infections,with an infection rate from 30 % to 90 %.DM and infections are two mutually-influenced diseases,thus DM complicated by infection may form a vicious circle.There's research showing that infection is the primary factor causing the death of diabetic patients.Therefore,it is necessary to lucubrate the characteristics and risk factors of DM complicated by infections,providing clues for taking timely and effective prevention and treatment measures.Objective By analyzing the elderly DM patients' infection prevalence,sites of infection and phase spectrum of bacterial infection,we want to investigate the characteristics and risk factors of DM concurrent Infection,which can provide some clinical support for the early detection,prevention and treatment of DM infection.Last but not the least,based on this,the preventive measures can be developed to reduce the morbidity of elderly DM patients' infection.Subjects and Methods The elderly(age ?60)2545 cases of DM patients who were under treatment in the People's Liberation Army(PLA)202 hospital during Jan.01,2014 to June 30,2015 are analyzed retrospectively,and 800 cases are infected while the rest 1745 cases are not infected.Collect the relevant clinical data,input computer,then statistically process the results using the SPSS22.0 statistical software.Divide the cases into infection group(800 cases)and non-infection group(1745 cases).Then the patient's age,sex,duration of DM,body mass index,fasting blood-glucose,glycosylated hemoglobin and complications of DM are analyzed univariately and multivariately using Logistic regression.Result 1.2545 cases of elderly patients with DM are enrolled,there are 800 cases(31.43%)with infection.In the infection group,536 cases are of respiratory infections(67.0%).Then 211 cases(26.38%)are of urinary tract infection,of which 107 cases are asymptomatic,accounting for 50.71% of it.97 cases(12.13%)have digestive infections,and the rest 59 cases(7.38%)have other parts of the infection(23 cases(2.88%)have skin soft tissue infections.);Among the 800 cases of infection,there are 114 cases complicating two or more parts,accounting for 14.25% of the infections.2.Infection Pathogen Bacterial Phase Spectrum: 441 pathogens are culture isolated,of which 53 pathogens are Gram-positive bacteria(G +),279 pathogens are Gram-negative bacteria(G-),93 pathogens are Fungus,and the rest 16 are other pathogens(2 are Acid-fast bacilli sputum smear-positive)3.Univariate analysis shows that sex,BMI,course of the disease,FPG,diabetic retinopathy,diabetic nephropathy,diabetic peripheral neuropathy,coronary heart disease,dyslipidemia,glucose-lowering regimen prior to admission are factors of elderly DM infection.Multivariate logistic regression analysis further suggests that gender(OR = 1.460 P <O.O01),FPG(OR = 1.060 P <0.001),diabetic nephropathy(OR = 2.595 P <0.001),coronary heart disease(OR = 1.385 P <O.01)and insulin hypoglycemic therapy(OR = 2.416 P <0.001)are the risk factors of elderly DM infection.Conclusion Firstly,elderly patients with DM are prone to infection,and the main sites of infection are respiratory system,urinary system,skin soft tissue and digestive system.Secondly,gender,FPG,diabetic nephropathy and coronary heart disease are risk factors of elderly DM infection.
