Background: Hypertension as an important cardiovascular disease risk factor in maintenance hemodialysis(maintenance hemodialysis MHD)patients can increase the rate of hospitalization and mortality.It has become an important problem for renal physicians to solve.The hypertension prevalence rate is especially high in MHD patients in primary hospital such as Zhaoyuan People's hospital of Shandong province(level 2 grade)and is difficult to control.The study of relationship between hypertension and hemodialysis adequacy in this kind of primary hospitals is lack so far.In this study,103 cases of hypertension in MHD patients of Zhaoyuan People's hospital of Shandong province have been selected.Though comparing hypertension and hemodialysis adequacy,the clinical study can provide more effective control of hypertension to reduce the rate of hypertension in MHD patients in basic-level hospitals.Purpose: By analysising the link of hypertension and hemedialysis adequacy in 103 cases of hypertension of MHD patients in Zhaoyuan People's hospital of Shandong province(level 2 grade),the study can provide evidence for effective control of hypertension.Methods: According to the urea clearance index(Kt/V)in the 103 MHD hypertension patients for 3 months,two groups have been divided:67 cases of adequate dialysis(Kt/V ?1.2)and 36 cases of inadequate dialysis group(Kt/V < 1.2).Two groups' hypertensions were compared by monitoring blood pressure for 12 consecutive times.The statistical analysis of two groups' hypertensions has been accomplished according to the systolic blood pressure(SBP),diastolic blood pressure(DBP)and the average of the mean arterial pressure(MAP)before,during and after hemedialysis.In the inadequate dialysis group(Kt/V < 1.2),the urea clearance indexes(Kt/V)of 23 cases have been improved up to 1.2 or higher by adjusting hemedialysis program.According to the adjustment of Kt/V,hypertensions of these 23 cases were compared by monitoring blood pressure for 12 consecutive times in 3 months.The statistical analysis of these hypertensions has been accomplished according to the systolic blood pressure(SBP),diastolic blood pressure(DBP)and the average of the mean arterial pressure(MAP)before,during and after hemedialysis again.Results: The control of hypertension which including the SBP,DBP and MAP before,during and after hemedialysis in 67 cases of adequate hemodialysis group is better than that in the 36 cases of inadequate hemodialysis.(P < 0.05).There are differences between hypertensions of 23 cases in inadequate hemodialysis group(Kt/V < 1.2)which achieved adequate hemodialysis(Kt/V?1.2)by adjusting hemedialysis(P < 0.05).Conclusion: Adequate hemodialysis can effectivly control hypertension in maintenance hemodialysis patients.
