Relationship Between Chinese Quality Of Life And Dialysis Adequacy In Maintenance Hemodialysis Patients

ObjectiveTo evaluate the effect of hemodialysis adequacy on the quality of life(QOL)of maintenance hemodialysis patients,guide the formulation and improvement of dialysis treatment plan and improve the QOL of maintenance hemodialysis patients.At the same time,this study is also a preliminary exploration and attempt to evaluate maintenance hemodialysis patients using the Chinese Quality of Life Instrument(CHQOL),with the help of the CHQOL,to fully explore all factors that may affect the QOL and dialysis adequacy of maintenance hemodialysis patients,laying a solid foundation for the future development and application of relevant scales based on the basic theory of traditional Chinese medicine.MethodIn this study,a cross-sectional study was conducted in patients who underwent regular hemodialysis in the Blood Purification Center of Xiyuan Hospital,China Academy of Chinese Medical Sciences from July 2019 to December 2019.Finally,81 patients who met the criteria were included.General data such as name,age,gender,dialysis age,primary disease,vascular access,and laboratory data such as serum calcium,serum phosphorus,serum PTH,serum albumin,hemoglobin,serum potassium,B-type natriuretic peptide(BNP)?and ?2-microglobulin(P2-MG)were collected in detail.According to the Kt/V value,the patients were divided into adequate dialysis group,patients with dialysis 3 times a week,Kt/V? 1.2 for adequate dialysis group,Kt/V<1.2 for inadequate dialysis group;patients with dialysis 2 times a week,Kt/V? 2.1 for adequate dialysis group,Kt/V<2.1 for inadequate dialysis group.The original data were processed by SPSS 22.0 statistical software combined with Excel.By comparing the differences in the general data and laboratory examination data of the inadequate dialysis group,the factors that may affect the adequacy of dialysis were explored and analyzed.The Chinese quality of life score of patients in each group was obtained using the CHQOL.The differences in the Chinese quality of life score in the inadequate dialysis group were compared under the conditions of different factors affecting the adequacy of hemodialysis,so as to focus on the relationship between the adequacy of hemodialysis and the Chinese quality of life.ResultsAccording to the value of Kt/V,the 81 patients included in this study were divided into the dialysis group(60 patients)and the dialysis group(21 patients).The highest score of Chinese quality of life score was 228,the lowest score was 127,and the average score was 176.79.The average score of the dialysis group was 179.70,and the average score of the dialysis group was 168.48.There were statistical differences between the two groups(P<0.05).1.The differences in general clinical conditions between the two groups were analyzed,and the results showed that:in terms of general information,there were no significant differences in age and age distribution,dialysis age and dialysis age distribution,primary disease type and vascular access type between the two groups(P>0.05),and there were statistically significant differences in gender distribution(P<0.01).In laboratory tests,two groups of patients with blood calcium levels and blood calcium,serum phosphorus levels and scope of concentration distribution,the values and their distribution,concentration of hemoglobin and serum PTH distribution,blood albumin concentrations and their distribution,mean the BNP and its distribution,blood beta 2-MG value before and after dialysis and its decline rate had no obvious difference(P>0.05),but the two groups of potassium concentration of mean value and the concentration distribution was statistically difference(P<0.05).2.Analysis of the quality of life of the general clinical situations score comparison,the results showed that the gender differences,age of dialysis,blood calcium level,the distribution of blood phosphorus concentration range,the value of serum PTH,concentration of hemoglobin and blood albumin and BNP level change in shape,spirit,mood and quality of life scores between had no obvious difference(P>0.05);There were statistically significant differences in age distribution in the areas of form,spirit and quality of life(P<0.01).There were no significant differences in the primary disease types in the fields of shape,emotion and quality of life(P>0.05),but there were significant differences in the field of god(P<0.01).There were statistically significant differences in vascular access in the morphological field(P<0.05),but no statistically significant differences in the mental field,emotional field and the total score of quality of life(P>0.05).There were statistically significant differences in serum potassium concentration in the morphological field and the total score of quality of life(P<0.05),but there was no significant difference in the scores of the spiritual field and the emotional field(P>0.05).In addition,there was a significant linear correlation between Chinese quality of life(QOL)and age and dialysis adequacy(P<0.01),which met the linear regression criteria(P<0.05).3.To evaluate the impact of dialysis adequacy on the scores of various dimensions of Chinese quality of life,the results showed that there were statistically significant differences in the shape field scores and the total score of quality of life between the sufficient and inadequate dialysis groups of MHD patients(P<0.05),while there were no significant differences in the scores of the mental and emotional fields(P>0.05).In terms of shape dimension scores,there were statistically significant differences in complexion and sleep between the two groups(P<0.05),while there were no significant differences in activity ability,diet and digestion(P>0.05).In the aspect of god dimension,there was no statistical difference between the two groups in climate adaptation,consciousness,thinking and total score of quality of life(P>0.05).In terms of seven emotion dimension,there were statistically significant differences in anger and fear between the two groups(P<0.05),while there were no statistically significant differences in language expression,sadness and happiness(P>0.05).Conclusion1.The CHQOL can better evaluate the general clinical situation of MHD patients.The higher the quality of life score,the better the general clinical situation of patients.2.Dialysis adequacy can affect part of THE Chinese quality of life score of MHD patients,and the score of patients with adequate dialysis is higher.
