Objective: To explore the clinical value of bile duct stones type?operation selection and choledochoscopy in the treatment of cholangiolithiasis,and observe Qingdan syrup clinical efficacy after bilestone operation.Methods: 102 cases of operative patients with choledochoscopy were retrospectively analyzed in Hubei provincial hosptial of traditional Chinese medicine from2010.03-2013.06.All patients were separately classified to different type and surgery method according to The Guideline of the Diagnosis and Treatments of Hepatoliasis(Guideline)established by Group of biliary tract surgery,Chinese Society of Surgery,Chinese Medical Association.In addition,102 patients were randomly divided into Qingdan syrup group(n = 59)and routine treatment group(n =43).Serum levels of bilirubin pro/post-operation,rates of short term postoperative complication,rates of postoperative residual stones,rates of postoperative symptom improvement and postoperative stone recurrence rates between two groups were statistically analyzed.Results: Based on the Guideline,there were 45 cases of type ?,34 cases of type ?,23 cases of type E,separately.Type ? were composed of 16 cases of ?a,17 cases of?b and one case of ?c.Type E were included by 18 cases of Ea and 5 cases of Ec.There were 58 cases of choledocholithotomy,32 cases of anatomically partial hepatectomy,and 12 cases of choledochojejunosfomy.Our results from 102 patients revealed that total rates of short term postoperative complication,rates of postoperative residual stones,rates of postoperative symptom improvement and postoperative stone recurrence rates orderly were 17.65%,10.78%,97.06%,8.82%.Compared with routine treatment group,postoperative stone recurrence rates in Qingdan syrup group significantly decreased(P<0.05).However,there were no significant differences in rates of short term postoperative complication,rates of postoperative residual stones and postoperative symptom improvement rates(P>0.05).In addition,serum levels of preoperative bilirubin from both groups were dramatically dropped in comparison of pre-operation(P<0.05).Moreover,serum levels of postoperative bilirubin in Qingdan syrup group were significantly lower than routine treatment group(P<0.05).Conclusion: 1.Surgical procedure is the most valuable therapy in the treatment of intra/extra-hepatic bile duct stones.Based on Guideline,optical operation selection and standardization use of choledochoscope can obtain good clinical results2.Qingdan syrup can significantly reduce jaundice symptom and serum levels of bilirubin levels in patients with bile duct stones after operation.3.Qing Dan syrup has satisfactory clinical efficacy in preventing recurrence of postoperative cholelithiasis.
