Analysis On The Surveillance Of Adverse Events Following Immunization In Jinan City,2012-2013

Objectives:1. The study is to analyze the occurrence features of Adverse Event Following Immunization(AEFI) in Jinan;2. Through the monitoring of the complete index of Adverse Event Following Immunization in Jinan, evaluation of the operation of the information management system of Adverse Event Following Immunization. 3. By analysis of the epidemiological characteristics of Adverse Event Following Immunization in Jinan City,and according to the different types of vaccines,the data do survival analysis with fever, local swelling and induration as the outcome variable in order to grasp the time distribution of reaction and provide guidance for the prevention of vaccination reactions that may occur4. By the incidence rate of Adverse Event Following Immunization, in order to evaluate the quality of the immunization services.Methods:The AEFI data were collected through AEFI information immunization of the China Information System.By using that generate excel2003 format file, and import SPSS19.0 software for collection and analysis, then use the descriptive analysis method to describe the AEFI epidemiological distribution characteristics. Chi square test compares the difference in the incidence of abnormal reaction between different vaccines.By the method of moment, from the inoculation to the reaction time interval distribution to normality test, with Kaplan Meier method analysis and comparison about the different types of vaccine in ending time interval of survival curves.Results:1. 1541 AEFI cases of 2012-2013 were reported.The reported rate within 48 h was 99.16%, and the investigated rate within 48 h was 90.00%. 2. 72.02% cases were ?1 years old and 3.05% cases were ?7 years old. The ratio of male and female was 1.25:1. 3. The first three vaccines are DTaP,MCV, MPV. 67.80% happened after the 1st dose and 70.86% within ld after vaccination. 99.03% AEFIs were cured or getting better.4. In the classification of AEFI cases,96.63% cases were common, minor reactions, 2.99% were Rare Adverse Reactions and others were <1%.Among 1541 AEFI cases, NIP vaccine accounted for 78.20%.The estimate reported incidence of NIP vaccines were 60.37~1053.50 per million doses. The most common Rare Adverse Reactions for NIP Vaccines is BCG Lymphadenitis. 5. DPT vaccine series after vaccination in half of swelling time differences in the theoretical value, which according to appear swelling as an ending, the DPT IPV and vaccine Hib most likely to occur, the second one is the DPT(no cell) vaccine and DT is the last one. Ma gills wind series vaccine after inoculation with half of fever reaction time differences in the theoretical value. With leprosy vaccine the most easy to fever reaction, ma gills wind vaccines take second place, finally for measles vaccine.Conclusion:1. The information management system of Adverse Event Following Immunization is in good working order, monitoring the sensitivity than there has been a significant increase from 2005 to 2011, but the report timeliness in 2013 did not meet the national requirements, therefore Jinan AEFI monitoring quality remains to be further improved.2. Adverse Event Following Immunization often crowd for small age group children, with national immunization programs vaccine and the first dose and high incidence of 1 day after inoculation, types and occurrence time, has been clear about the crowd the vaccine as a monitoring point.3. Polio vaccine related cases of abnormal reaction rates 3.35/(1 million doses), higher than the WHO. The results showed that polio attenuated vaccine cause polio vaccine related disease is 0.40/(1 million doses), so we should strengthen the use of the IPV.4. After inoculation DPT IPV and Hib immovable vaccine the most prone to redness, attention needs to be improved; For easy to local induration after inoculation of the DPT vaccine series, this study found that the series of scleroma occurred after vaccination no statistical difference(P=0.287); It is not easy to import the MMR vaccines fever reaction, can strengthen is recommended.5. From 2012 to 2013, no preventive inoculation accident in Jinan, which shows vaccination service quality is ok.
