Objective To investigate the predictive ability of amylase in the peritoneal drainage of pancreaticoduodenectomy for pancreatic fistula at different time periods,so as to provide a reference for early detection of pancreatic fistula after pancreatoduodenectomy.Methods The clinical data of 106 patients with pancreaticoduodenectomy performed in the First Affiliated Hospital of Xinjiang Medical University from June 2014 to June 2018 were retrospectively analyzed.To investigate the relationship between amylase in abdominal drainage fluid and early postoperative complications within 10 days after operation.Results: Pancreatic fistula occurred in 24(22.6%)of 106 patients with PD in this group,including 18 cases of grade A,4 cases of grade B,and 2 cases of grade C.Univariate analysis found that the age,tumor diameter,pancreatic texture,and diameter of the main pancreatic duct in patients with PD were related to pancreatic fistula after PD(P <0.050).Multivariate logistic regression analysis found that the diameter of the main pancreatic duct ? 3 mm was pancreatic after PD.Independent risk factors for fistula(OR= 4.856,P = 0.004).On the 5th,7th,and 10 th days after PD,the amylase level of the peritoneal drainage fluid had a good diagnostic effect on pancreatic fistula(grade A + B +C).,0.948,POD5(AUC = 0.885),POD7(AUC = 0.897),POD10(AUC = 0.938)in the prediction of B + C pancreatic fistula.Conclusions The diagnostic efficacy of amylase level in the abdominal drainage fluid on pancreatic fistula on day 5,7 and 10 after PD is better,especially on the 10 th day after operation.
