Objective: To investigate factors for lumbar foraminal stenosis(LFS)by analyzing the structure of the intervertebral foramen.To explore the main evaluation index for lumbar foraminal stenosis(LFS)in parasaggital MRI finding at L4-5 and L5-S1 Segments between Preoperatively and the final follow-up,Visual Analogue Scale(VAS)and Oswestry disability index(ODI)were used To Evaluate the clinical efficacy of transforaminal lumbar fusion surgery(TLIF).Materials and methods: From Jan 2014 to Dec 2015,30 patients(observation Group,group A)who were admitted to the Second Affiliated Hospital of Dalian medical university,with the LFS that were confirmed by MRI and surgery,retrospectively analysis MRI finding and clinical efficacy in group A,compared with control group including 32 cases(control group,group B)whose lumbar intervertebral foramen imaging examination showed no abnormalities at the same period.All patients had complete relevant imaging examination,including the lumbar spine X-ray,CT and MRI.To observe intervertebral foramen at sagittal plane,selected T2W1 sequence.Measuring three important indexes of intervertebral foramen(foraminal height,foraminal width and foraminal area)at L4-5 and L5-S1 at Parasaggital MRI finding,then got the efficacy by Image processing software.The observation group underwent TLIF,record VAS(waist pain,leg pain),ODI in preoperative and final follow-up.Using SPSS 17.0 software for statistical analysis,target factors including gender composition ratio,age,intervertebral foramen indexes,Gender distribution using ? 2 test,other data using t test,If P <0.05,the difference was statistically significant.Results: 1.Gender distribution showed no significant difference between group A and group B,P> 0.05;age distribution showed no significant difference between group A and group B,P> 0.05.2.comparison results of the evaluation index between group A and group B at L4-5,L5-S1 intervertebral foramen segments are as follows:(1)group A L4-5,L5-S1 foraminal height less than group B,the difference was statistically significant(P <0.05);(2)group A L4-5,L5-S1 foraminal width less than group B,the difference was statistically significant(P <0.05);(3)group A L4-5,L5-S1 foramen area less than group B,the difference was statistically significant(P <0.05).3.Compared with that preoperatively,VAS(waist pain,leg pain)and ODI of group A at final follow-up,results are as follows:(1)VAS(waist pain)showed significant improvement(P<0.05).(2)VAS(leg pain)showed significant improvement(P <0.05).(3)ODI showed significant improvement(P <0.05).Discuss: Many pathologic factors can cause LFS,such as soft tissue,bone tissue and radiculopathy,these factors often coexist.foraminal height,foraminal width and the foraminal area can regard as the main evaluation indexes for LFS in parasaggital MRI finding.TLIF can significantly relief low back pain,leg pain and other symptoms in patients with LFS,clinical efficacy is satisfied.
