Analysis Of The Causes Of Bad Oral Fixed Prosthesis And Retreatment

Purposes: Nowadays,however,there are many failed cases in clinical operation that caused gingivitis,periapical inflammation,loosen abutments,bad breath which may have severe physiologically and mentally impact on patients.Bad prosthesis has become a problem that urgently needs to be addressed.This article will analyze the cause of bad prosthesis from several aspects as well as the possibilities of the retreatment to the patients that affected by bad prosthesis.Objective: Analyse the cause of bad prosthesis and its complications,the possibilities of retreatment.Data: Data collected from 54 cases where patients who had bad prosthesis that sought medical treatment in Dalian Hospital University First Subsidiary Hospital Department of Stomatology during the period from August 2014 to January 2016.Method: By collecting patients' comments,histories of present illness and clinical examination,then completing bad prosthesis questionnaires,analyzing the causes,removing and retreating bad prosthesis and 12-24 months follow-up visits.Lastly,assess the outcome of retreatment by patient satisfactory survey.Outcome: Among 54 patients who were affected by bad prosthesis,there were15 males(38 teeth),39 females(86 teeth),totaled to 124 teeth(12 anterior teeth,48 premolars,64 odontoprisis).Aged from 26 to 80,averaged to 50.7years old.Treatment history ranged from 2 to 28 years.13 patients had cast crowns and bridges,25 used ceramic,full-ceramic bridges and crowns were used to treat 2 patients and 14 used cast bridges.39 of them had gums symptoms,35 of 54 patients had abutment pain,22 patients were having periodontal disease,2 of them had broken abutments,10 cases where the prosthesis had fallen off,loosened or broken.15 patients had bad breath and 2had oral mucosa diseases.The retreatment had been conducted on 26 patients,10 had prosthesis re-stabilized and restored.Patient did not feel pain after the restoration and the gums were healing well.Dark shadow around the root mostly disappeared,patient could chew properly and the shapes of the prosthesis were beautiful.The other 16 patients had the bad prosthesis and abutments removed,revisits after 3 months revealed that the gums were recovering properly and the alveolar bones were mostly healed so that the patients could receive the removable partial denture treatment.Conclusion:1.Bad prosthesis is commonly caused by treatment principal violation,poorly designed dentures and inappropriate techniques used in manufacturing dentures.2.Bad prosthesis could cause gingivitis,periapical inflammation,loosen abutments,bad breath etc.Long-term use of bad prosthesis could lead to the dysfunction of abutments that may affect the patients severely.3.The choice of re-treatments for patients who have bad prosthesis should base on their abutments and economic conditions.
