Early Low Level Of Chlorine On The Impact Of Post-Stroke Epilepsy

Objective:The aim of the present study was to explore influence factors of PSE and analyze the clinical characteristics and prognosis of PSE.Method: From January 2009 to January 2016,there were 2088 hypertensive intracerebral hemorrhage(ICH)patients and 2833 atherosclerosis cerebral infarction(CI)were hospitalized at the Department of Neurology and Neurosurgery in the First Hospital of Dalian Medical university.According to the inclusion and exclusion criteria,a total of 500 PSE cases and 320 Stroke patients were enrolled.There were 363 male patients and 137 female patients with their ages ranged from 39 to 85 years(mean 62.13±10.35 years)in PSE cases.There were 204 male patients and 116 female patients with their ages ranged from 41 to 88 years(mean 63.37±10.50)in Stroke cases.The PSE were divide into EarlyOnset Seizures(EOS)and Late-Onset seizures(LOS)according to the time of epileptogenesis.There were 216 male patients and 93 female patients in EOS cases.There were 131 male patients and 60 females patients in LOS cases at the same time.The basic information and biochemistry items were registered for all patients.All patients had been followed up by clinic or phone for a mean of 4 years.Data were analyzed with the Statistical Package for Social Sciences for Windows software(version 20.0,SPSS).A 2-tailed value of P<0.05 was considered significant.Results:1.The incidence of EOS was lower than LOS.The incidence of PSE in CI was lower than it in ICH,the difference were statistically significant(P=0.000<0.05).2.In CI patients,the incidence of EOS was lower than it in ICH,the difference were statistically significant(P=0.000<0.05).In CI patients,the incidence of LOS was lower than it in ICH,the difference were statistically significant(P=0.000<0.05)3.Patients in EOS exhibited lower level of chlorine,lower level of calcium,the proportion of cortical involvement,more cerebral lobes involvement,large lesions,the temporal involvement,the parietal lobe involvement when compared with Stroke patients without seizures by univariate analysis(P=0.001,0.000,0.000,0.000,0.000,0.000,0.000,0.000,separately).In Binary logistic regression analysis,EOS were independent and significantly associated with low level of chlorine(OR=0.888,P=0.000),cortical involvement(OR=3.270,P=0.040),large lesions(OR=2.738,P=0.000),the temporal involvement(OR=1.854,P=0.020).4.Patients in LOS exhibited lower level of chlorine,the proportion of cortical involvement,more cerebral lobes involve ment,large lesions,the temporal involvement,the parietal lobe involvement,the occipital lobe involvement when compared with Stroke patients without seizures by univariate analysis(P=0.000,0.000,0.008,0.000,0.000,0.017,0.009,separately).In Binary logistic regression analysis,LOS were independent and significantly associated with low level of chlorine(OR=0.878,P=0.000),large lesions(OR=4.119,P=0.000),the temporal involvement(OR=3.406,P=0.001).5.In PSE patients,the Seizures forms of EOS were difference from LOS(P=0.000<0.05);the Seizures forms of cerebral infarction(CI)were difference from intracerebral hemorrhage(ICH)(P=0.000<0.05).6.In EOS patients,the total incidence of LOS was 75.44% and the incidence of CI had no statistical differences with ICH(P= 0.142>0.05).Conclusion:1.Cortical involvement,large lesions,the temporal involvement were independent risk factors for EOS.2.Large lesions,the temporal involvement were independent risk factors for LOS.3.EOS patients were more likely to have LOS.4.The early low level of chlorine was independent risk factors for PSE.
