Objective: To investigate the effect of NBI-ME on the diagnosis of chronic atrophic gastritis and intestinal metaplasia, and the effect of IHb on the diagnosis of chronic atrophic gastritis by using ordinary white light endoscopy, magnifying endoscopy(ME) and NBI magnifying endoscopy(NBI-ME).Method: According to the corresponding inclusion and exclusion criteria, patients selected in our hospital from December 2013 to June 2015, diagnosed by ordinary endoscopy, ME, and the NBI-ME. Lesions clarity at NBI-ME to pit typing were evaluated. Epithelial surface pale blue ridge-like structure(light blue crest LBC) were observed. Corresponding lesion common white endoscopic gastric mucosa within the hemoglobin index(IHb) values in the antrum, gastric body and suspected lesion biopsy for pathological examination were measured.Results: 1. Compare the lesions of clarity, NBI-ME was better than ME(P = 0.03), and NBI-ME and ME were better than the conventional white light endoscopy(*** P <0.001).2. Chronic atrophic gastritis sensitivity of diagnosis of NBI-ME was 82.81% and a specificity of 87.50%, compared with the ME under the diagnosis of chronic atrophic gastritis sensitivity, both of which were significantly different(P = 0.006). ME diagnosis of chronic atrophic gastritis sensitivity of 64.06% and a specificity of 76.78%, significantly better than ordinary white light endoscopy of 46.88% and 58.93%, the difference was statistically significant(P = 0.009, P = 0.016)3. LBC on intestinal metaplasia diagnosis rate was 88.33%, sensitivity of 86.05% and specificity of 89.61%. LBC distribution range differences in the degree of intestinal metaplasia has a significant correlation occurred, the difference was statistical significance(** P <0.01, χ2 = 9.66).4. Chronic atrophic gastritis mucosal IHb mean of(37.2 ± 10.32), the non-atrophic gastritis mucosal IHb mean of(76.81 ± 11.42), difference between themselves had statistical significance(P <0.05).Conclusion: 1. Clarity of observation Lesion of NBI-ME, significantly better than ME, ordinary white light endoscopy, chronic atrophic gastritis diagnostic sensitivity and specificity compared with ordinary white light endoscopy has improved significantly.2. LBC can be used as diagnosis of intestinal metaplasia characteristic performance under NBI-ME, guide targeted biopsies.3. IHb microscopic measurement values in ordinary white light, can provide a reference for judging whether benign gastric mucosal lesion localized presence of atrophy.
