Objective: To investigate the relationship between blood pressure variability and carotid artery intima media thickness and their influence on the prognosis of acute ischemic stroke with different image types.Method: Continuously select 136 cases of hospitalized CIS patients in the Affiliated Hospital of Wannan Medical College from September 2014 to June 2015.Register the basic conditions of the patient.Make iconography classification basing on the DWI result.Refer to IMT as the cutoff point,divide the patients into IMT<1mm group,1mm ? IMT < 1.5mm group and IMT ? 1.5mm group.At the same time,use ABPM(ambulatory blood pressure monitoring)to monitor blood pressure,choose 24 h,daytime and nighttime SSD(systolic blood pressure standard deviation)and DSD(diastolic blood pressure standard deviation)represents the variability of BPV(blood pressure variability),analyze the relationship between IMT and BPV,and study the effect on prognosis of different iconography classification of CIS.Results: 1.The frequency of hypertension increases in the IMT normal group,thickening group and plaque group.Compared with the normal IMT group,the age and FGB of IMT group and plaque group are higher,the difference is statistically significant(P<0.05).And there were no significant differences in gender,diabetes,coronary heart disease,smoking,UA,TG,LDL,HDL and other groups(P>0.05).Compared with the normal IMT group,IMT thickening group and carotid artery plaque group 24 h SSD,24h DSD,d SSD,d DSD,n DSD value were large,the difference is statistically significant(P<0.05).Compared with IMT group,the 24 h SSD value of plaque group is larger,the difference is statistically significant(P<0.05).2.According to correlation analysis between IMT and the time(24 hours,day and night)of mean blood pressure and BPV,it is observed that IMT and 24 h SBP,d SBP,24 h SSD,24 h DSD,d SSD are positively associated,relationship with 24 h SBP(r=0.403,P = 0.008),24 h SSD(r = 0.487,P = 0.017)is strong.3.The poor prognosis group compared with good prognosis group,age,hypertension,diabetes,24 h SSD,24 h DSD,d SSD,d DSD,FGB difference had statistical significance(P<0.05).Were no statistical significance of each risk factor,the results suggest that age,hypertension,diabetes,24 h SSD,24 h DSD,d SSD,d DSD,FGB are the influence factors for acute CIS patients of 6 months.To continue the Logistic regression analysis,find that only 24 h SSD,d DSD is associated with prognosis of 6 months,relations between the factors and prognosis of the rest is not obvious,therefore,only 24 h SSD,d DSD are the undesirable independent factors for the 6 months prognosis of acute CIS Patients.4.Compared with posterior circulation infarction group,the 24 h SSD of the former cycle cortical infarction group and the basal ganglia infarction group increase,and corona radiata infarction and posterior circulation infarction area,higher d SSD of basal ganglia infarction group,the difference was statistically significant(P <0.05).Compared with single infarction,multiple focal infarction 24 h SSD,24 h DSD,d SSD,n SSD,IMT were higher than in single infarction group,the difference was statistically significant(P<0.05).Conclusion: 1.The average blood pressure and blood pressure variability in patients with acute ischemic stroke is associated with an increased carotid intima-media thickness.2.24 h systolic blood pressure variability and daytime diastolic blood pressure variability is independent impact factors for the prognosis of patients with acute ischemic stroke.3.Different type image credits of different blood pressure variability in patients with acute ischemic stroke.
