| Objective: Ankylosing spondylitis(AS),Rheumatoid arthritis(RA)and osteoarthritis(OA)were all common diseases that manifests as inflammatory arthropathy.AS was a kind of chronic inflammatory disease characterized with sacroiliitis,enthesitis and synovitis,and closely related to HLA-B27.RA is a systemic disease that manifests as symmetric polyarthritis.And OA is a kind of arthritis caused by joint degenerative.The pathogenesis and prognosis of AS,RA and OA are essentially different.It was found that many immune cells and cytokines played important roles in the pathogenesis of rheumatoid arthritis and ankylosing spondylitis.At the same time the immune cells and cytokines in the incidence of OA participation degree and the depth of the now more and more attention.Many research confirmed that tumor necrosis factor alpha(TNF-α),interleukin-1(IL-1)and interleukin 6(IL-6)were important cell factors involved in immune response and immune regulation.The aim of this study was to investigate the similarities and differences in synovitis by measuring the serum and synovial fluid levels of AS、RA and OA patients.Methods: This was a comparative study of data from 60 patients(20 AS,20 RA and 20 OA),that were diagnosed in the Hebei Province Cangzhou traditional Chinese Medicine Hospital from Jan 2015 to Jan 2016.The levels of TNF-α,IL-1 and IL-6 in serum and synovial fluid were determined by avidin biotin peroxidase complex enzyme-linked immunosorbent assay(ABC-ELISA).The clinical data of patients were collected such as age,disease duration,erythrocyte sedimentation rate(ESR),c-reactive protein(CRP),etc.The levels of TNF-α,IL-1 and IL-6 were compared and analyzed among the different groups.All the data were analyzed by SPSS17.0 for windows Statistical software.The mean number±standard deviation(x ± s)was used to express the measurement data.The t test was adopted for comparison between groups.Linear correlation was used to in analyse.P value<0.05 was considered significant..Results:1 The serum levels of TNF-α,IL-1 and IL-6 in AS and RA group were significantly higher than OA group(P<0.01).And the serum levels of TNF-α,IL-1 and IL-6 in AS group were significantly higher than RA group(P<0.01).2 The synovial fluid levels of TNF-α,IL-1 and IL-6 in AS group were significantly higher than OA group(P<0.01).And the synovial fluid levels of TNF-αand IL-1 in RA group were significantly higher than OA group(P<0.01).The synovial fluid levels of TNF-αand IL-6 in AS group were significantly higher than RA group(P<0.05).3 The serum levels of TNF-α,IL-1 and IL-6 in AS,OA and RA group were significantly lower than synovial fluid(P<0.01)4 There was no significant correlation between ESR,CRP and IL-1,IL-6,TNF-αin patients with AS,RA and OA.Conclusion:1 TNF-α,IL-1 and IL-6 play important roles in the pathogenesis of synovitis in AS、RA and OA,especially AS and RA.2 The levels of TNF-α,IL-1 and IL-6 in synovial fluid were significantly higher than serum in patients of inflammatory arthropathy.It suggested that the synovium of joint maybe the main part of the inflammatory lesions.3 IL-6 maybe play more important role in the pathogenesis of OA than TNF-α and IL-1. |
