Objective: To observe the efficacy and safety of ALA-PDT therapy in the treatment of moderate and severe acne with different intensity of light intensity guided by PpIX.Method: 72 cases of acne patients with 5% aminolevulinic acid(ALA)2H packet darkroom.Data collection and quantitative analysis of protoporphyrin tX(Pp IX)lesions in different fluorescence intensity.According to the PpIX fluorescence intensity and PDT illumination intensity random,single blind divided into 4 groups,namely,A group(high illumination intensity strength,high PpIX group),B group(high illumination intensity strength,low PpIX group),C group(low illumination intensity,high PpIX group),D group(low illumination intensity,low PpIX group).Routine ALA-PDT treatment,once a week,a total of 3 times,and follow up treatment for fourth weeks,eighth weeks about the effectiveness and safety situation.Acne grading system GAGS(global)evaluation of the effectiveness of each visit as a record of an interview.Safety indicators included erythema,swelling pain,reactive acne,pigmentation,facial itching,and the duration of the above indicators,and the severity score was analyzed.Result: 1 Effectiveness: before and after treatment in each group at the 4th week,8th week gags integral compared with baseline improvement was statistically significant,group A,group B,group C pairwise had no statistical difference,group D and group A,group B,group C had effective reduction,the difference is statistically significant;Safety: after the first treatment,each side incidence rate was not statistically significant,but erythema,swelling and pain continued time in group C(low illumination intensity and high PpIX group)and group D(low illumination intensity and low PpIX group)and group A(high illumination intensity,high PpIX group),group B(high illumination intensity and low PpIX group)reduction was statistically significant.After the second and third treatment,the incidence of erythema and swelling pain and duration in the C group(low illumination intensity,high PpIX group),D group(low illumination intensity,low PpIX group)were statistically significant.There was no significant difference in facial itching,reactive acne and pigmentation after treatment.In addition,in three treatment,C group(low illumination intensity,high PpIX group),D group(low illumination intensity,low PpIX group)adverse reaction severity decreased with statistical significance.Conclusion: High PpIX fluorescent intensity patients should be treated with low illumination intensity ALA-PDT treatment,while the low fluorescence intensity of PpIX in patients with high illumination intensity ALA-PDT;PpIX fluorescence intensity guiding improved ALA-PDT therapy can obtain the same clinical efficacy,but with lower adverse reactions and clinical promotion prospects.
