Objective : Compare risk factors in women with papillary thyroid carcinoma and medullary carcinoma with respect to cancer risk factors and auxiliary clinical diagnosis while aiming at early prevention.Method:1.The clinical data of 462 consecutive patients with pathological diagnosis of thyroid cancer was collected between 2013 and 2015.Exclusion criteria were patients with Nodular goiter and Hashimoto’s thyroiditis,hyperthyroidism,on oral anti-thyroid drugs.Grouping was done according to the histological types of thyroid carcinoma and classified into two groups: thyroid papillary carcinoma(PTC)group and medullary thyroid carcinoma(MTC)group.2.Data was statistically analyzed with SPSS 17 and the following assessed: age,body mass index,smoking history,history of diabetes,gynecological history,thyroid cancer family history,TG,TSH,FT3,FT4,TG,anti TPO.12 parameters had no statistical significance.Categorical data was analyzed by the chi squared test(x2)test,while measurement data was compared with independent samples t-test and by multivariate logistic regression analysis.Results:1 PTC group had an average age 46+12 years old while the average age of MTC group was 53+16 years old.The average body mass index in patients with PTC was 25.2+5.01Kg/m2 and in the MTC group it was 26.1+3.22 Kg/m2;175 cases(37.9%),were smoking patients with 148 cases(37.9%)in the PTC group and 27(37.5%)in the MTC group.126 patients had the diagnosis of diabetes(27.3%),including 105 cases of PTC patients(26.9%)and 21 cases(29.2%)of MTC patients.57 patients had a positive gynecological disease history(12.3%),including 48 cases of PTC patients(12.3%)and9 patients with MTC(12.5%).There was a family history of thyroid carcinoma in 47cases(10.2%)with 39 patients with PTC(10%)and 8 patients with MTC(11.1%).Hormone measure levels showed PTC patients with the mean value of TG to be 65.5 +21.43 ng/ml,and MTC patients with the mean value of TG to be 16 + 12.34 ng/ml;Patients with PTC had mean value of TSH 1.45 + 0.69 m IU/L,and patients with MTC had mean value of TSH 1.98 + 0.99 m IU/L;PTC patients had mean value of FT3 of 4.73+ 2.37 pmol/L,and for MTC group to be 4.58 + 0.37 pmol/L;PTC patients had mean value for FT4 to be 15.4 + 2.59 pmol/L,and MTC patients had mean value of FT4 was15.9 + 2.06 pmol/L;PTC patients had Anti-TG mean of 77.6 + 137.27 U/ml,and MTC patients had a mean 44.5 + 50.86 U/ml;PTC patients had Anti-TPO mean of 88 +245.61 U/ml,and The mean of the MTC group was 263 + 43.54U/ml.2.Analysis of test results showed that smoking,diabetes mellitus,gynecological disease history,family history of thyroid cancer in thyroid papillary carcinoma and thyroid medullary carcinoma group had no significant difference(P greater than 0.05,not statistically significant).Body mass index,FT3,FT4,anti TPO mean between the thyroid papillary carcinoma and thyroid medullary carcinoma group also had no significant difference(P greater than 0.05,no statistically significant).3.Age,TG and anti-TG in papillary thyroid carcinoma group had a higher mean than the medullary thyroid carcinoma group(P values less than 0.05,statistically significant);TSH in papillary thyroid carcinoma group had a lower mean that the medullary thyroid carcinoma group(P values less than 0.05,with statistical significance).4.Multivariate Logistic regression analysis showed that age and TSH were the independent factors for thyroid papillary carcinoma and medullary thyroid carcinoma.Conclusion:1.This study concluded that age and TSH were the independent influencing factors of thyroid papillary carcinoma and medullary thyroid carcinoma.2.Papillary thyroid cancer occurs in middle-aged women while medullary thyroid cancer occurs more in elderly women.Statistical results show that the thyroid papillary carcinoma under the age of medullary thyroid carcinoma;this and the seventh edition of surgical science results are consistent.3.The statistical results showed papillary thyroid carcinoma TSH group were lower than the medullary thyroid carcinoma group.4.Age and TSH are two indicators which can assist in the clinical diagnosis of papillary thyroid carcinoma and medullary thyroid carcinoma.
