Objective: To explore the influence of diabetes mellitus(DM)on CAT score,pulmonary function,blood gas analysis and times of hospitalization due to acute exacerbation last year in chronic obstructive pulmonary disease(COPD)in-patient.By comparing the differences of above index among healthy people,COPD patients,patients with COPD combined with DM to explore the influence of DM on COPD,so that it can provide a reference when we assess the severity of the disease in clinical work and provide the basis for controlling of COPD comorbidities to delay the progression of COPD,improving the quality of life,improving the prognosis of patients with COPD.Materials and methods: Collect the records of 75 cases of patients with COPD who hospitalized in department of respiration in the second affiliated hospital of Dalian medical university in 2015.1 to 2015.10,including40 cases of COPD patients as COPD group and 35 cases of COPD patients combined with DM as COPD combined with DM group,divided into COPD combined with DM≤10 years subgroup(less than or equal to 10 years)(n = 21)and COPD combined with DM>10 years subgroup(more than 10years)(n=14)according to the prolonged duration of DM.At the same time collect 36 cases of healthy people as the control group.Recorded the age,sex,weight,prolonged duration of COPD(75 cases of COPD),smoking index,used CAT quality of life scale for symptomatic assessment,underwent pulmonary function testing including FVC,FEV1,FEV1 / FVC,FEF25%-75%,blood gas analysis including PaO2 and PaCO2,at the same time recorded the times of hospitalizations due to acute exacerbation last year of 75 COPD patients.SPSS17.0 software was used for statistical analysis to compare among the control group,the COPD group,COPD combined with diabetes group and COPD combined with DM≤10 years subgroup and COPD combined with DM>10years subgroup about the differences of CAT score,pulmonary function,blood gas analysis and times of hospitalizations.In this study,all analyses were carried out with the SPSS statistical package,version 17.0,measured data with mean± standard deviation,the normal distribution data between the two groups were compared using independent-samples t test,non normal distribution data using nonparametric test,count data,which was recorded as % using chi-square test,A p value <0.05 was considered significant.Results: Compared to the control group,the CAT scores and PaCO2 of COPD combined with DM group and COPD group are higher,FVC,FEV1,FEV1/FVC,FEF25%-75%,PaO2 were lower,P<0.01.FVC,compared to the COPD group,PaO2 in the COPD combined with DM group were significantly lower,CAT scores,PaCO2 were significantly higher(P < 0.05),especially CAT scores,FVC,PaO2 were the most significant(P < 0.01),the other index liked FEV1,FEV1/FVC,FEF25%-75%,times of hospitalizations due to acute exacerbation last year between the two groups were not statistically significant(P> 0.05).Compared to the COPD combined with DM≤10 years subgroup,the CAT scores in the COPD combined with DM>10 years subgroup was higher,PaO2 was lower,(P<0.05),the other index liked FVC,FEV1,FEV1/FVC,FEF25%-75%,PaCO2,times of hospitalizations due to acute exacerbation last year between the COPD combined with DM>10 years subgroup and the COPD combined with DM ≤10 years subgroup were not statistically significant(P> 0.05).The number of ABCD group of COPD group was 6,17,13,4respectively,owned 15.0%,42.5%,32.5%,10.0% respectively.The number of ABCD group of COPD combined with DM group was 5,14,8,8 respectively,owned14.3%,40.0%,22.9%,22.9% respectively,there was no statistically significant in the ratio of ABCD group between the two groups.Conclusion: DM has a certain influence on symptomatic assessment,pulmonary function and blood gas analysis in COPD in-patient,symptomatic assessment and arterial partial pressure of oxygen are more obvious in COPD patients who have a prolonged duration of diabetes more than ten years.
