Objective : To detect the serum complement C3,C4 concentration in acute cerebral infarction(ACI)patients and the affection of repetitive limb ischemic(RLI)on the concentration of serum complement C3,C4 and the degree of neural function defect.To explore the influence of RLI on the ACI and the possible mechanism.Methods :1.According to the inclusion and exclusion standard,87 patients with cerebral infarction were selected as the case group,and the normal-control group were consisted of 65 healthy person.Then the case group were randomly divided to the case-control group(47cases)and RLI group(40 cases)based on contrast research method,All conform to the comparability in age and gender composition.2.The serum complement C3 and C4 levels were detected by the IMMAGER 800 automatic special protein analyzer produced by American BECKMAN COULTER and supporting C3,C4 reagent kit,The rate scatter nephelometry were used to measure.completed by the Department of clinical laboratory;the First Affiliated Hospital of Dalian Medical University.3.The indexes were detected and recorded before and after treatment,and the degree of neural function defect were the National Institute of Health stroke scale(NIHSS).The data before treatment were marked as “1”and the date after treatment were marked as “2”,and subtraction was marked as “D-value”.Meanwhile observe andrecord the untoward effect during RLI operation.SPSS 20.0 was used and P<0.05 were considered statistically significant.Results:1.The serum levels of complement C3-1、C3-2 of the case group(1102.390±195.405 、 1166.559 ± 215.853)were significantly higher than the normal-control group(1012.820±244.346)(P<0.01,P<0.01);The levels of serum complement C4-1of the case group(235.366 ± 53.423)were higher than the normal-control group(224.115±63.124),but with no statistical significance;The case group levels of serum complement C4-2(254.574±67.184)were significantly higher than the normal-control group(P<0.01).2.The level of serum complement C3-1 of the case group was significantly positively correlated with the two NIHSS score(2.620±1.014,2.150±1.206)(r =0.489,P<0.01、r =0.446,P<0.01).And the level of serum complement C3-2 of the case group was significantly positively correlated with the two NIHSS score(r =0.355,P<0.01、r =0.382,P<0.01).In the case group,Serum complement C4-1 and NIHSS1 score was positively correlated,but with no statistical significance.The level of serum complement C4-1 was significantly positively correlated with NIHSS2 score(r =0.219,P<0.05).The serum complement C4-2 was positively correlated with the two NIHSS scores,but with no statistical significance.The serum complement C3D-value(-83.697±174.451)、complement C4 D-value(-16.910±37.133)was negatively correlated with the two NIHSS score,but with no statistical significance.The serum complement C3 D-value 、 C4 D-value were positively correlated with the NIHSS D-value(0.46±0.86),but with no statistical significance.3.The NIHSS1 scores has no significantly difference in case control group and RLI group(2.550±0.746,2.700±1.265).The NIHSS2 scores in RLI group(1.850±1.231)were lower than in case control group(2.400±1.136),the difference has statistically significance(P<0.05).The NIHSS D-value in the RLI group(0.830±0.781)were bigger than the NIHSS D-value in case control group(0.150 ± 0.807),the difference was statistically significance(P<0.01).The NIHSS2 scores in case control group were lower than the NIHSS1 scores,but with no statistical significance.TheNIHSS2 scores in RLI group were significantly lower than the NIHSS1 scores,the difference has marked statistically significance(P<0.01).4.The serum levels of complement C3-1 and C3-2,C4-1,C4-2 has no significantly difference in case control group(1102.830±160.411、1198.694±199.855、234.872±50.418、255.528±71.410)and RLI group(1101.800±236.964、1116.261±234.426、236.029±57.960、253.200±62.002).The serum complement C3 D-value in the RLI group(-25.391 ± 131.839)were signicantly higher than the serum complement C3 D-value in case control group(-124.722±191.296)(P<0.05).The serum complement C4 D-value in the RLI group(-7.840±24.269)were higher than the serum complement C4 D-value in case control group(-25.778 ± 44.877),the difference has marked statistically significance(P<0.01).The serum levels of complement C3-2in case control group were higher than the serum levels of complement C3-1,the difference has marked statistically significance(P<0.01).The serum levels of complement C4-2 in case control group were higher than the serum levels of complement C4-1,the difference has marked statistically significance(P<0.01).The serum levels of complement C3-2in case control group were higher than the serum levels of complement C3-1,the difference has no statistically significance.The serum levels of complement C4-2 in case control group were higher than the serum levels of complement C4-1,the difference has no statistically significance.5.All of the patients accepted RLI treatment,36 patients expressed the numbness of the operated limb,8 patients felt ache and 1 patient got punctuate hemorrhage at the same time.But all disappeared within 10 minutes after the treatment and the punctuate hemorrhage was disappeared within 2 day.Conclusion:1.The levels of serum complement C3、C4 significantly increased in acute cerebral infarction patients.It promoted that complement C3 、 C4 might take part in the pathophysiologic mechanism of the acute cerebral infarction.2.The levels of serum complement C3、C4 can partly evaluate the severity of theacute cerebral infarction.And the higher the level of serum complement C3、C4 incate a more serious neurologic deficits.3.RLI can alleviate the degree of neurologic deficits in acute cerebral infarction patients,may through inhibiting the increase of the level serum complement C3、C4.That indicate RLI can improve the condition of acute cerebral infarction patients,may be a safe and effective method for the acute cerebral infarction patients.
