Clinical Analysis Of Surgery Correction In Patients With Uncompensated Sagittal Imbalance By Using The Operation Of Posterior Subtraction Osteotomy

Objective: To analyze surgery correction in patients with uncompensated sagittal imbalance by using the operation of posterior subtraction osteotomyMethods: 20 patients(male 6,female 14)with uncompensated sagittal imbalance were adopted by the department of spine surgery of the second hospital of Dalian Medical University from January 2013 to December 2015,who were thoracolumbar kyphosis by after through posterior subtraction osteotomy.The average age was 66.1±7.59 years(range 55 to 80 years).The total spine X-ray and CT were used to evaluate the related indicators of sagittally balanced such as TK?LL?PI?PT and SVA were collected both preoperative and postoperative.ASIA nervous functional classification and examination of VAS had been done.Postoperative appearance,the improvement of both life and work and the occurrence of complications were also evaluated.Results: All the 20 patients were operated remedial surgery.They were no death?paralysis and infection after operations.The ache of the patients in both waist and back had been alleviated or even vanished.The value of TK,LL,PT and SVA were improved apparently after surgery,there were significant differences in statistical analysis(P<0.01).The examination of VAS had debased significantly after surgery(P<0.01).The difference between preoperative and postoperative of VAS was significantly related to the difference between preoperative and postoperative of all indicators of sagittally balanced such as TK?LL?PI?PT and SVA(P<0.01 or P<0.05).The ASIA nervous functional classification after the operation was better than it before the operation.And the postoperative appearance and quality of life had been improved apparently.Conclusions: The operation of posterior subtraction osteotomy was proved to be a kind of really safe and reliable surgery to cure uncompensated sagittal imbalance.There were many advantages such as,less operation bleeding,shorter operation time and lower rate of complication et al.
