BackgroundCoronary Atherosclerotic Heart Disease(CHD)was a common cardiovascular related diseases,it was closely related to the way of life,in recent years,as the improvement of people's living conditions,the incidence of coronary heart disease was rising year by year and was becoming the important public health problem of the 21 st century.As a result,the relative risk factors of Coronary Atherosclerotic Heart Disease was very important,and so were the pathogenesis,clinical diagnosis and prognosis of CHD.Serum cystatin C had been known as an indicator of renal function.While the latest studies suggest that serum cystatin C had a close connection with CHD and the results were not consistent as well.In this paper,the correlation of serum cystatin C and CHD was discussed.ObjectivesThe aim of the study was to investigate the correlation between serum cystatin C and clinical classification of CHD,to investigate the association between serum cystatin C and branches of pathological coronary artery,to investigate the connection between serum cystatin C and degree of coronary artery stenosis,and to evaluate the value of serum cystatin C in diagnosing coronary heart disease.Methods160 patients with Coronary Atherosclerotic Heart Disease who were going to undergoing coronary angiography(CAG)or percutaneous coronary intervention(PCI)surgery in Anyang City People's Hospital from May 2014 to December 2015 were enrolled to the study.According to the degree of stenosis of coronary angiography,The subjects were divided into two groups.35 patients(stenosis <50%)were divided into control group.The other 125 patients(stenosis ?50%)were divided into CHD group.Collect the information of these patients such as age,gender,height,weight,blood pressure level and serum cystatin C,glucose,lipids,and renal function data.Besides,according to the clinical types,severity of coronary artery lesions,and branches of pathological coronary artery,the other 125 patients were divided into different groups.The levels of each group's serum cystatin C were compared.Results1.There was no statistical significance between the control group and the CHD group in the following aspects such as age,gender,trigryceride and total cholesterol levels,left ventricular ejection fraction,smoking history and the duration of diabetes.There was statistical significance between the control group and the CHD group in the following aspects such as low density lipoprotein,cholesterol,apolipoprotein B,high density lipoprotein cholesterol and apolipoproteinA1.Besides,the level of low density lipoprotein and cholesterol was higher in the CHD group than in the control group.The level of high density lipoprotein cholesterol and apolipoproteinB was lower in the CHD group than in the control group.2.According to clinical classification of coronary heart disease,the level of serum cystatin C in AMI group was higher than in SAP group,UAP group and control group(P <0.05).The level of serum cystatin C in UAP group was higher than in SAP group and control group(P <0.05).The level of serum cystatin C in SAP group was higher than in control group(P <0.05).3.According to the branches of pathological coronary artery,The level of serum cystatin C in multivessel disease group was higher than in double vessel disease group,single-vessel disease group,and control group(P <0.05).The level of serum cystatin C in double vessel disease group was higher than in single-vessel disease group and control group(P <0.05).The level of serum cystatin C in single-vessel disease group was higher than in control group(P <0.05).4.According to the degree of coronary artery stenosis,The level of serum cystatin C in severe stenosis group was higher than mild stenosis group,moderate stenosis group and the control group(P <0.05);The level of serum cystatin C in moderate stenosis group was higher than mild stenosis group and control group(P <0.05);The level of serum cystatin C in mild stenosis group was higher than control group(P <0.05)5.According to the level of serum cystatin C,as the level of serum cystatin C was getting higher the following index such as age,Gensini points,branches of pathological coronary artery,Creatinine,urea,uric acid were getting higher as well(P <0.05).6.Through the analysis of Pearson correlation analysis,we found that the level of serum cystatin C was positively related to the following index such as age,low density lipoprotein cholesterol,gensini score,branches of pathological coronary artery,(Correlation coefficient r = 0.275,0.225,0.732,0.658,P<0.005).The level of serum cystatin C was negatively related to the level of high density lipoprotein cholesterol(Coefficient coefficient r =-0.178,P =0.025<0.05).The level of serum cystatin C wasn't relevant to triglyceride,total cholesterol,serum creatinine,urea nitrogen,uric acid.(Coefficient coefficient r =-0.33?-0.076?0.007?0.000?0.118,P <0.05).7.In order to predict coronary artery disease according to the level of serum cystatin C,we draw the receiver operating characterisitic(ROC)curves,which suggested that the best cut off value was 1.215 mg / L,and the area under the ROC curve was 0.717.Its sensitivity was 67.9%,Its specificity was 76.2%.Conclusions1.It might be possible for serum cystatinC to be used as a predictor of coronary heart disease.2.The level of serum cystatin C had a close connection with coronary heart disease in the following aspects such as the clicinal classicification and the branches of pathological coronary artery.3.As the increasing of age,the level of serum cystatinC seemed to be higher for the patients who were suffering coronary heart disease.
