BackgroundThe post ischemic stroke depression is one common affective disorder in Neurology.Currently,the diagnosis of this disease mainly depends on clinical manifestations and related neurological scores.These atypical depressive symptoms or mild depressive symptoms does not cause the doctors enough attention,resulting in misdiagnosis.When the disease has progressed to a certain extent,the effect of drug treatment would be poorer than early treatment.So,there would be great help to the treatment of depressive if a laboratory examination can be found to make early diagnosis of depression.There are many studies on the pathogenesis of this disease.Neurotransmitter theory is gradually being recognized by most experts and scholars.The original theory has progressively been found,such as anatomical factors,inflammatory factors,genetic and so on.To the patients who have postpartum depression,some studies have found that there was a certain decrease of immunization levels than the healthy.The nervous system has a close relationship with the immune system.Ischemic injury to the nervous system caused dysfunction of the immune system as well as the patient's disability.As T lymphocyte is a major player in cellular immunity,these changes of immune system manifested by T lymphocytes and its subsets.Contact between depressive symptoms and cellular immunity has gradually been understood.ObjectiveTo investigate the relationship between the change of 0T lymphocyte subsets in peripheral blood and post ischemic stroke depression.MethodsThe patients who definitely diagnosed ischemic stoke were selected from December 2014 to June 2015,all subjects performed the corresponding imaging examination,and gived the routine department of internal medicine treatment.According to?Hamilton depression scale,HAMD-17?,all the patients were divided into two groups,one complicated with depression group includes 30 patients whose HAMD-17 score no less than 7,the not complicated with depression group includes 30 patients whose HAMD-17 score less than 7,the clinical data?gender,age,smoking,alcohol,chronic diseases,location of acute infarct?and neurological scores?National Institute of Health stroke scale,NIHSS;Activities of daily living,ADL,Brathel;Mini-mental State Examination,MMSE?were compared and analysised.Participants also included 20 healthy persons.Flowcytometry was used to detect the subsets(CD4+,CD8+,CD4+/CD8+)of T lymphocyte in the peripheral blood of the three groups.Results1.Compared to the not complicated with depression group,the complicated with depression group had similar clinical data and neurological scores?p>0.05?;2.The level of CD4+ in complicated with depression group?40.90±7.01?% was significantly lower than healthy controls?49.90±7.62?% and not complicated with depression group?45.73±8.23?%?P < 0.05?;The level of CD8+ in complicated with depression group?27.27±8.29?% was increased compared with healthy controls?21.50±5.04?%?P<0.05?;The levels of CD4+/CD8+ in healthy controls,complicated with depression group,not complicated with depression group were?2.45±0.69?,?2.01± 0.75?,?1.60±0.42?,the differences in the three groups were all statistically significant?F=11.01,P<0.05?.ConclusionsThere is a certain degree of immune imbalance in the patients after ischemic stroke,which is more significance in the patients complicated wih depression,the immune imbalance mainly manifested by the reduction of CD4+ in the auxiliary T lymphocytes.
