ObjectiveThis study focus on monitoring and researching the condensate water in the ventilator pipe in the intensive care unit,in order to analyze the time when condensate water contaminated was most likely to occur and explore the influence factors, and for the sake of providing reference for the clinical treatment that how to control the condensate water contaminated to the medical personnel.MethodsWe enrolled 86 patients who were admitted into the intensive care unit(ICU)of Jinzhou Central Hospital and received mechanical ventilation(MV)of more than 12 hours from September 2014 to October 2015.The condensate water in the ventilator intake pipe was sampled at 4 hours,8 hours and 12 hours after the ventilation began. At the same time, measuring and recording the temperature difference of the gas temperature after in wet in the pipe and ward temperature(temperature difference),length of pipe, minute ventilation(VE)and the relative humidity of gas in wet(relative humidity of gas)in the pipe by using relevant instruments at 12 h.Using statistical software SPSS 17.0 for statistical analysis.Counting the contaminated cases among the three time points and using the chi square test to compare the infection rate,we can find the most vulnerable period. The four factors were analyzed by correlation analysis and multiple linear regression analysis, then building the regression model in order to find out the influence factors affecting the condensate water contaminated.Results1.The comparison of the contaminated rates of condensate water at three time point(1)The condensate water at the 4h?8h and 12 h after the ventilation began all are contaminated, and the rates of contaminated are 7.0%?62.8%?75.6%;there is a statistical difference(?2=91.654,P<0.05)comparing the three infection rates.(2)Further comparison between two of the three rates: The contaminated rate at 8h(62.8%)is higher than at 4h(7.0%),and there is a statistical difference(?2=58.971,P<0.017);The contaminated rate at 12h(75.6%)is higher than at 4h(7.0%),and there is a statistical difference(?2=83.494,P<0.017)too;The contaminated rate at 12h(75.6%)is higher than at 8h(62.8%),however,there is no statistical difference(?2=3.300,P>0.017).2.The correlation analysis of the condensate water contaminated level and factorsThe condensate water contaminated level in the ventilator pipe are correlated with temperature difference of the gas temperature after in wet in the pipe and ward temperature(temperature difference),length of pipe, minute ventilation(VE)and the relative humidity of gas in wet in the pipe(P<0.05),and the correlation coefficient are 0.728?0.338?0.637 and 0.717.3.The regression analysis of the condensate water contaminated level and factorsMultiple linear regression analysis shows that temperature difference of the gas temperature after in wet in the pipe and ward temperature(temperature difference),minute ventilation(VE)and the relative humidity of gas in wet in the pipe are predictable factors of the condensate water contaminated level.Multivariate regression model is Y?=-69.938+9.092X1+5.721X2+0.870X3(X1 =temperature difference,X2 = minute ventilation,X3 = relative humidity of gas),and the regression model has statistical difference(F=35.519,P=0.000).ConclusionCondensate water contaminated in the ventilator pipe is most likely to occur between 4h and 8h after the ventilator begins, nursing staff should pour out the condensate water before the time that the condensate water is contaminated and the first time to clean up the condensate water in the pipe is no more than 4h.Temperature difference of the gas temperature after in wet in the pipe and ward temperature(temperature difference),length of pipe, minute ventilation(VE)and the relative humidity of gas in wet(relative humidity of gas)in the pipe are the influence factors, and medical personnel can control the condensate water contaminated considering the four influencing factors and other clinical conditions. The predictable factors of the condensate water contaminated level include temperature difference of the gas temperature after in wet in the pipe and ward temperature(temperature difference),minute ventilation(VE)and the relative humidity of gas in wet in the pipe, and the regression model shows that the greater the temperature difference, the greater minute ventilation(VE)and the greater the relative humidity of gas,the higher the contaminated level of the condensate in the pipe.
