Objective:Observe the influence of local drug delivery with aminomethylbenzoic acid in wound cavity to hemorrhage after total hip arthroplasty,then evaluate the effectiveness and safety of hemostatic effect of aminomethylbenzoic acid.Methods:Prospective randomized controlled trial is adopted in this paper.It collects the information of 90 patients who accepted the unilateral and initial total hip arthroplasty from March 2014 to December 2015 among them 55 males and 35 females are included.They are divided into 3 groups randomly according to different drug uses in wound cavity: 30 cases in Group A,dissolve 3 pieces of aminomethylbenzoic acid injection(10ml:0.1g)in 20 ml normal saline and then inject into articular cavity retrogradely through drainage tube after closing the incision;30 cases in Group B,dissolve 2 pieces of tranexamic acid powder injection(0.5g)in 50 ml normal saline and then inject into articular cavity retrogradely through drainage tube after closing the incision;30 cases in Group C,inject 50 ml normal saline into articular cavity retrogradely through drainage tube after closing the incision.Clip the drainage tubes in the three group for 4 hours after operation and then collect the evaluation index after operation such as,drainage quantity 48 hours after operation,number of blood transfusion people,hemoglobin and HCT on the first and third day after operation,etc.Observe the patients to confirm whether clinical symptoms of Deep Venous Thrombosis in lower limbs and Pulmonary Embolism are occurred.Results:The selected 90 patients have finished the whole therapeutic process.There is no statistical significance(P>0.05)of the comparative difference of the three groups patients before operation in gender,age,height,weight,BMI,3species of diseases,hemoglobin before operation and HCT.Drainage quantity and blood transfusion number after operation in group A and group B is less than group C(P<0.05);there is no obvious difference of blood transfusion number,hemoglobin after operation and HCT in group A and group B(P>0.05).All patients in the three groups have no deep venous thrombosis in lower limbs and pulmonary embolism.Conclusion:Local drug delivery with Aminomethylbenzoic Acid in wound cavity can reduce the drainage quantity and blood transfusion number obviously after total hip arthroplasty;the influence level of aminomethylbenzoic acid and tranexamic acid on reducing the drainage quantity and blood transfusion number is similar without increasing the risk of deep venous thrombosis in lower limbs and pulmonary embolism.Aminomethylbenzoic Acid is a hemostasis method which is worth using widely as its low cost and high safety in wound cavity.
