| Objective: To study the relationship among microscopical performance of Colorectal polyp based on Narrow-band Imaging endoscopy(NBI)technology and the distribution characteristics of TCM syndrome types as well as the pathological features,in order to provide clinical evidence for clinical application of Chinese medicine Differential Treatment.Methods: Chose 100 patients that met inclusion and diagnostic criteria and confirmed by endoscopic and pathologic diagnosis in the spleen-stomach disease in-patient department and clinic of our hospital of from September2014 to September 2015,and then divided into different groups by syndrome differentiation,in order to observe distribution characteristics of TCM syndromes under the NBI endoscopic and analyze the relationship between pit pattern,pathological pattern and type of syndrome.Results:(1)There were 56 male patients with colon polyps(56%)and44 female patients with colon polyps(44%),the number of male patients with colon polyps were 1.27 times than that of female.Colorectal polyps located mostly in sigmoid(32%),followed by rectum(29%)and least in ileocecal lesions(3%).Mostly clinical syndromes were seen with diarrhea and constipation.(2)Under NBI mode,the main open type of gland tube of colon polyps was type pit-Ⅰ,type pit-Ⅱand type pit-Ⅲ.Morphology by YamadaⅠ,Yamada II and Yamada Ⅲ are the most.Type pit-Ⅰmore appears in Yamada Ⅰand Yamada II.Type pit-II more appears in Yamada Ⅰand Yamada II.Type pit-Ⅲ more appears in Yamada Ⅲ and Yamada II.Typepit-Ⅳ more appears in Yamada Ⅲ and Yamada Ⅳ.Main pathological type was the type of inflammatory polyp and tubular adenoma.(3)The open type of gland tube of colon polyps had no correlation with the sex.(4)TCM syndrome types of colon polyps mainly included four types:spleen deficiency and blood stasis type(37%),the resistance to hot and dampness stasis type(34%),the liver depression and qi stagnation type(15%),the internal organ cold type(14%).(5)Colon polyps in TCM tongue and pulse:The main tongue characteristics were big fat tongue and soft tongue,the main tongue coating were white,the color of the tongue were light white and light red,the main pulse condition were slow and deep.(6)There was no relationship between TCM syndromes and sex,diameter(P > 0.05),but they related to age,However,the resistance to hot and dampness stasis type was mostly found in patients aged from 40 to 49 and 50 to 59,while spleen deficiency and blood stasis type was mostly found in patients over 60 years old,Deficiency of spleen and kidney was mostly found in patients over 70 years old.(7)The relativity between the open type of gland tube of colon polyp with TCM syndrome classification was positive(P < 0.05).(8)The relationship between TCM syndromes and pathological pattern: Inflammatory polyp,Hyperplastic polyp and Juvenile polyp were mostly seen in the type of resistance to hot and dampness stasis,tubular adenoma was mostly seen in the type of spleen deficiency and blood stasis.Villous adenoma and tubular villous adenoma were mostly seen in the type of The deficiency of spleen kidney Yang.Conclusions:This study suggests:(1)The prevalence rate of colorectal polyps was higher in men than women.It mostly located in the rectum and sigmoid colon.(2)Colon polyps in TCM tongue and pulse prompted that the nature of colorectal polyps was mainly intermingled deficiency and excess,and the main pathogenesis was dampness,phlegm,heat and blood stasis.(3)This main type of TCM syndrome was followed by pleen deficiency with blood stasis,Dampness-heat obstruction,and Qi stagnation and phlegm coagulation,Deficiency of spleen and kidney.(4)There was high consistency between pathological diagnosis and pit patterns.(5)The relativity between the open type of gland tube of colon polyp with TCM syndrome classification was positive,in order to provide clinical evidence for clinical application of Chinese medicine Differential Treatment. |
