Objective: To explore the clinical pathological characteristics and CT imagingfinding in 45 to 59 years old midlle-aged patients with lung cancer,and to summarize and evaluate the role of CT imaging.Methods:1 Study Objects: To retrospectively analyse 600 middle-aged and 450 ?60 years old patients with lung cancer cases in the second affiliated hospital of Hebei North University from December 2010 to December 2015.These patients had been verified by biopsy or pathological.The pathological types,clinical and imaging data were retrospectively analysed.In 600 middle-aged lung cancer cases,499 cases were peripheral lung cancers,101 cases were central lung cancers.2 Analyze contents:(1)Clinical symptoms and physical signs such as cough,hemoptysis,coughing of phlegm,blood-stained sputum,chest pain,choking sensation in chest,asthma,metastatic symptom,irritating,fever.(2)The CT imaging were interpreted by three experienced radiologists respectively.The radiology festures contain sublob sign,burrs sign,bronchial truncation or stenosis,vessel convergence sign,pleural indentation sign,zigzag sign,cavity,air sign,air bronchogram sign,segmental no tention,lymphangitic spread of carcinoma,hilar mass,obstructive pneumonia,obstructive no tention,grinding glass sign or holo sign,hilar or mediastinal lymph node enlargement,pleural effusion.3 Contrast analysis: Contrastively analyzed the different pathological types,tumor cell differentiation and CT imagingfinding in midlle-aged patients withlung cancer,Results: 1Clinical results:(1)Clinical symptoms and physical signs in middle-aged patients such as cough(X~2=11.68,P<0.05)hemoptysis(X~2=10.62,P<0.05)?coughing of phlegm(X~2=9.39,P<0.05);blood-stained sputum(X~2=9.08,P<0.05)?chest pain(X~2=1.12,P>0.05)?choking sensation in chest(X~2=0.98,P>0.05)?asthma,metastatic symptom(X~2=2.15,P>0.05)?irritating(X~2=1.02,P>0.05)?fever(X~2=0.22,P>0.05)?metastatic symptom(X~2=0.38,P>0.05).2 CT imaging findings: There is no difference between central lung cancers and peripheral lung cancers as to soft tissue mass(P=0.192)?bronchial truncation or stenosis(P=0.178)?obstructive no tention sign(P=0.182),but sublob sign(P<0.05)and burrs sign(P<0.05)are more common in peripheral lung cancers than in central lung cancers.(3)There were 499 patients withperipheral lung cancer and 101 with central bronchogenic carcinoma in middle-aged patients.Central bronchogenic carcinoma cases were arised from lung cancer in last century,while the ratio of central lung cancers cases in our cases,it is obviously decreased.There is a high ratio in peripheral lung cancer cases and a relatively low ratio in central bronchogenic carcinoma cases.(4)Among 600 middle-aged lung cancer patients,there were 420 adenocarcinoma,98 squamous-celled carcinoma,69 small cell lung cancer,120 bronchiole alveolar carcinoma,5 adenocarcinoma and bronchiole alveolar carcinoma,10 large cell carcinoma,1 huge cell carcinoma,3 adenocarcinoma and squamous-celled carcinoma.Among 600 middle-aged patients,there were 302 lesions with poor differentiation,101 lesions with low-modera differentiation,98 modera differentiation,32 high-modera differentiation,67 high differentiation.Conclusion:1 Cough?hemoptysis?coughing of phlegm and blood-stained sputum have a high positive ratio in middle-aged patients.2 Central lung cancer cases incidence in middle-aged patients was significantly lower than elderly patients while peripheral lung cancers incidence increased relatively in middle-aged patients.3 Soft tissue mass?bronchial truncation or stenosis ?obstructive no tention sign are the main features of central lung cancers.Sublob sign and burrs sign are the main features of peripheral lung cancers;There is no difference in mediastinal lymph node enlargement and pleural effusionbetween central lung cancers and peripheral lung cancers.4 Adenocarcinoma are most common in middle-aged lung cancer patients,and there is usually poorly differentiated adenocarcinoma.Middleaged patients with lun cancer often metastasize when they are diagnosed.
