Objective:Retrospective analysis clinical data that were diagnosed with lung cancer from December 2010 to January 2014 of Yan’an University Affiliated Hospital diagnosed in Middle-aged and elderly patients. Through statistical analysis, investigate the clinical characteristics of middle-aged and elderly patients with lung cancer, expectingthe help that for the early diagnosis of Lung Cancer. Methods:(1) obtaining 260 cases clinical data that from December 2010 to January 2014 in Yan’an University Affiliated Hospital and diagnosed through cytology or pathology diagnosed middle-aged patients and elderly patients with lung cancer. In which the middle-aged group(45-59 years old) 128 cases(49.23%); the elderly group(60 years) 132 cases(50.77%).(2)Retrospective analysis the clinical data: gender, smoking index, the first clinical manifestations, performance CT of chest, the type of histological and clinical staging.(3)Deal with the data through SPASS19.0 statistical software, count data using chi-square test, with P <0.05 was considered statistically significant. Results:1. The gender distribution The middle-aged group: male 82 cases(64.1%), 46 females(35.9%); the elderly group: 94 males(72.7%) and 36 females(27.3%), both gender distribution no significant difference(p> 0.05), are more common in men. 2. Smoking indexThe middle-aged group with lung cancer in Smoking index ≧ 400, 62 cases(48.4%), Smoking index ≧ 400, 64 cases(48.5%), there was no significant difference of Smoking index(p> 0.05) in elderly patients with lung cancer in both groups, both have a higher smoking History.3. Clinical manifestationsThe middle-aged group of clinical manifestations: cough, 95 cases(74.2%), shortness of breath 52 cases(40.6%), chest pain, 34 cases(26.6%), the other,14 cases(10.9%), hemoptysis 8 cases(6.3%), asymptomatic 1 patients(0.7%); The elderly group of clinical manifestations: cough, 86 cases(65.2%), shortness of breath, 42 cases(31.8%), chest pain, 20 patients(15.2%), the other 22 cases(16.7%), hemoptysis 10 cases(7.6%), asymptomatic 2 cases(1.5%); two patients had no significant differences.4.The pathological types The middle-aged group: small cell carcinoma 26 cases(20.60%), adenocarcinoma 50 cases(39.10%), Squamous cell carcinomas 46 cases(36.10%), the other 6 cases(4.2%); The elderly group: small cell 26 cases of cancer(19.40%), adenocarcinoma 40 cases(30.20%), squamous cell carcinoma and 50 cases(38.10%), the other 16 cases(12.30%). Both pathological types were no significant differences(p> 0.05), are adenocarcinoma, squamous cell high.5. The clinical stage(NSCLC): the middle-aged group Ⅰ ~ Ⅲa of 54 cases(64.2%), Ⅲb of ~ Ⅳ 30 cases(35.71%); The elderly group of lung cancer patients Ⅰ ~ Ⅲa of 46 cases(51.11%), Ⅲb s ~ Ⅳ 44 cases(48.89%), clinical stage two groups were statistically no significant difference(p> 0.05).6. The middle-aged group Chest CT findings: around 40 cases(31.3%), central 88 cases(68.7%); The elderly group Chest CT manifestations: around 14 cases(10.67%), central 118 cases(89.4%), the statistical analysis, two chest CT showed a significant difference, namely the middle-aged group of peripheral lung cancer incidence rate higher than the older group, but they are the center of the high incidence of lung cancer(p <0.05). Conclusion:Results of the study showed that in the elderly lung cancer in gender distribution, with smoking index, the first clinical manifestation, pathological type, there was no significant difference in clinical staging(non-small cell lung cancer), and other aspects; performance CT of chest, the two have significant differences, namely middle-aged patients with lung cancer, "peripheral" incidence of lung cancer than the elderly more common, but they are mainly in central lung cancer.
