The Clinical Pathway Medical Risk Cost Estimation Based On Reliability Theory

With the development of social economy, the medical service quality expectations of China people are improving, the hospitals are facing great risk manage pressure. In addition, in the condition of medical health system reform and single disease system, extra medical treatment cost caused great cost pressure to hospital. However, traditional hospital management was lack of medical risk control measures and medical risk cost measures, many hospitals were in losing money condition, when they implemented single disease system, what's more, low medical service quality and patient satisfaction were big problems. Therefore, the study of clinical pathway risk cost analysis and risk cost estimation have great significance to improving medical security level and reducing medical risk cost. Through the current research of medical risk cost, a new single disease clinical pathway risk cost estimate method based reliability theory was developed, and an actual case was used in the paper. The main research contents are as follows:(1)Medical risk factors analysis of clinical pathway based fault tree analysis. First, introduce medical risk factors analysis steps of clinical pathway was introduced, and we analyzed esophageal cancer surgery clinical pathway. And then establish esophageal cancer surgery fault tree model. Finally, the risk reasons, risk influence, and control measures of esophageal cancer surgery clinical pathway were analyzed.(2) Medical risk cost estimate of clinical pathway model based on failure mode and effect analysis. Traditional study of medical risk cost only mainly paid attention to complications cost, and ignored the risk cost of clinical pathway. To solve this problem, a new inventory method based on reliability analysis was developed. First, fault tree analysis and failure mode and effects analysis was used to analyze risk factors in clinical pathway. And then combining with average cost of resolve and risk score to calculate the risk cost of clinical pathway. Finally, through an example in a hospital of Beijing verifies that the method proposed by this paper can evaluate risk cost of Clinical Pathway effectively.(3) Due to there were many problems of medical risk cost in our country, such as, low medical risk control level, lack of medical security cost control measures, Lack of risk management concept.
