Objective1.Based on the retrospective investigation of hospitalized cases of chronic obstructive pulmonary disease(COPD) in our hospital for nearly three years,distribution characteristics of Traditional Chinese Medicine(TCM) syndromes of COPD were analyzed,and that provide a basis for making and optimizing TCM clinical syndrome differentiation treatment of COPD.2.Observing the influence of Qibai Pingfei Capsule on energy metabolism of COPD model rats,and exploring the mechanism of action of Qibai Pingfei capsule to improve energy metabolism of COPD model rats from aspects of serological indicator and energy transformation of diaphragm,and that provide a laboratory basis and gist for clinical application of Qibai Pingfei capsule.Methods1.Through consulting relevant literature,clinical retrospective survey of COPD was developed,and data of 423 COPD patients were collected from putting general information,symptoms and signs of TCM of the survey as survey items,and correlative factors were made a statistical analysis,thus distribution characteristics of TCM syndromes of COPD were induced.2.The syndrome of phlegm and blood stasis obstructing lung of COPD model were made by the method of combination of disease and syndrome.120 male SD rats were randomly divided into 6 groups:normal group,model group,high-dose group of Qibai Pingfei,middle-dose group of Qibai Pingfei,low-dose group of Qibai Pingfei,and control group of nicorandil,with 20 in each group.In addition to normal group were fed normally,other groups were made model for 6 days a week,and modeling was suspend in the 7th day, for 4 weeks.During models making period,high-dose group,middle-dose group and low-dose group of Qibai Pingfei were fed 10ml/kg of liquid of Qibai Pingfeicapsule everyday(containing 100 mg,50mg,and 25 mg of Qibai Pingfei capsule per milliliter respectively),and control group of nicorandil were fed liquid of 10ml/kg of nicorandil(containing 0.3mg per milliliter).After the modeling,materials were took intensively.The lung function of rats were detected.Levels of serum glucose,albumin,triglyceride were detected by full automatic biochemical analysis tester,and serum leptin level were detected by ELISA,and ATP,ADP,AMP level of diaphragm were detected by high performance liquid chromatography.Results1.In the 423 COPD patients,men accounted for 349 cases, and women accounted for 74 cases;In the age distribution,the age of 18 to 39,40 to 59,60 to 79,80 and over accounted for 2,31,275,115 cases,respectively,and the largest proportion is the age of60 to 79,accounting for 65.0% of all cases,followed by the age of 80 and over,accounting for 27.2%;the largest proportion of cases of course is the course of 10 to 20 years(38.8%),followed by below 5 years(20.8%).2.Compared with normal group,the value of lung function parameter of rats in model group dropped significantly(P<0.01).Compared with model group,lung function parameter values in other groups were significantly improved(P<0.01).Compared with normal group,glucose in serum in model group,middle-dose group,and low-dose group of Qibai Pingfei elevated significantly(P<0.01);Compared with model group,glucose in serum in high-dose group of Qibai Pingfei and control group of nicorandil dropped significantly(P<0.01),and middle-dose group,and glucose in serum in low-dose group of Qibai Pingfei higher than model group,with no significant difference.Compared with normal group,serum albumin,triglyceride levels in model group dropped significantly(P<0.01);Compared with model group,serum albumin,triglyceride levels in other groups showed high level in the different degree.Compared with normal group,serum leptin level in model group and middle-dose group of Qibai Pingfeielevated significantly(P<0.05),and serum leptin level in high-dose group of Qibai Pingfei and control group of nicorandil had no significantly difference with norm group;Compared with model group,serum leptin level in high-dose group of Qibai Pingfei and control group of nicorandil dropped significantly(P<0.05),and serum leptin level in middle-dose group of Qibai Pingfei and model group had no significantly difference. Compared with normal group,serum T3 level in model group and middle-dose group of Qibai Pingfei dropped significantly(P<0.01);Compared with model group,serum T3 level in high-dose group of Qibai Pingfei and control group of nicorandil elevated significantly(P<0.05).Compared with normal group,serum TSH level in model group,low-dose group of Qibai Pingfei and control group of nicorandil dropped significantly(P<0.05);Compared with model group,serum TSH level in high-dose group of Qibai Pingfei elevated significantly(P<0.05).Compared with normal group,level of TP and ADP of diaphragm in model group dropped(P<0.05);Compared with model group,level of TP and ADP of diaphragm in Qibai Pingfei group increased in the different degree(P<0.05).The level of AMP has no significant differences between groups.Conclusion1.Syndrome of TCM of COPD can be divided to 6 type of syndrome,there are syndrome of phlegm-heat lying depressed in the lung,phlegm-damp amassing in lung,phlegm and blood stasis obstructing lung,qi deficiency of lung and kidney,water overflowing due to yang deficiency,and exterior cold and interior stagnant fluid.2.Qibai Pingfei capsule can well adjust and improve energy metabolism imbalance in COPD model rats of syndrome of phlegm and blood stasis obstructing lung.
