Prevalence Of Hyperuricemia In The Middle-aged And Elderly People And Association Between Serum Gamma-glutamyl Transferase Levels And Hyperuricemia:A Cross-sectional Study

Objective To investigate the prevalence of hyperuricemia in the middle-aged and elderly people in Sichuan Luzhou and association between serum gamma-glutamyl transferase levels and hyperuricemia. Methods A cross-sectional population survey for hyperuricemia was performed in Sichuan Luzhou(2014).Questionnaire,physical examination and biochemical detection were conducted in 4038 subjects. Results Total of 3675 eligible subjects were included.The prevalence of hyperuricemia was 33.3%( 396/1189) in males,higher than in females 25.02%(625/2486),the average prevalence of hyperuricemia was 29.2% in Sichuan Luzhou.Compared with normol group,participants in the hyperuricemia group in females showed significantly increased fasting plasma glucose(FPG), postprandial 2 h plasma glucose(2h PG),hemoglobin A1C(Hb A1c), aspertate aminotransferase(AST), alkaline phosphatase,(ALP),gamma glutamyl transpeptidase(GGT), serum creatinine(Cr),dyslipidemia, bady mass index(BMI), blood pressure(BP), waist-to-hip ratio(WHR)and age.Contrastly, FPG, Hb A1 c, high-density lipoprotein cholesterol(HDL-C) were decreased significantly in participants with hyperuricemia in males, while, AST, ALP, GGT, triglycerides(TG) were increased.Additionally, those of the high uric acid group exhibited a significantly higher prevalence of type 2 diabetes,obesity,hypertension,low HDL-C and hypertriglyceridemia in females, and low HDL-C,hypertriglyceridemia in males. Pearson correlative analyses were shown that serum GGT was positively correlated with FPG, 2h PG, Hb A1 c, ALT, AST, ALP,TC, TG, LDL-C, UA, Cr,neck circumference, BMI, SBP,DBP, WHR,but negatively correlated with HDL-C in females. It had also been found that serum GGT was positively correlated with 2h PG, AST, ALP,TC,TG,but negatively correlated with age in males.From the lowest to the highest quartile of serum GGT levels, the prevalence of hyperuricemia increased significantly(13.4%,19.9%,30.0% and 38.5% in females; 24.7%,29.6%,38.3%and40.8% in males).Binary logistic regression analysis showed that compared with subjects in the lowest quartile of serum GGT level, the adjusted odds ratios(ORs) in the highest quartile was 2.241 [95% confidence interval(CI), 1.598-3.141] in females and 1.763(95%CI, 1.180-2.634) in males. Conclusions The prevalence of hyperuricemia in the middle-aged and elderly people was high in Sichuan Luzhou, the prevalence of obesity, diabetes and dyslipidemia was significantly higher in subjects who had elevated uria acid levels, and association between uria acid and plasma glucose had some deferences between women and men.Besides, from the lowest to the highest quartile of serum GGT levels, the prevalence of obesity, diabetes, dyslipidemia and hyperuricemia increased significantly. Furthermore, GGT was deeply correlated with the risk of hyperuricemia, suggesting that it could potentially be used as predictive biomarkers for the hyperuricemia,especially, in females.
