Abstract/Summary:
Inflammatory bowel disease(IBD) is a group of chronic and idiopathic intestinal diseases characterized by periods of remission and recurrent relapses.IBD includes Crohn’s disease(CD) and ulcerative colitis(UC).Evaluating the activity of IBD timely and accurately is crucial to guide treatment.The activity of disease is reflected mainly by neutrophil granulocyte infiltration and epithelial damage, therefor the change of neutrophils in the intestinal tissue is correlate closely with the activity of IBD. Fecal calprotectin represents 60% of cytosolic proteins in granulocytes. The amount of calprotectin in feces is proportional to the neutrophil migration to the gastrointestinal mucosa. Endoscopic scores is considered as the gold standard method for detecting inflammatory activity in IBD.However, endoscopy is invasive,expensive and uncomfortable. Fecal calprotectin are charactered as high stability at room temperature, resistance to degradation and a homogenous distribution in stool and as well a strong correlation with endoscopic score.[1] There has been a Meta-analysis concerning the diagnostic accuracy of FC in assessing IBD inflammatory activity.Objective: To evaluate the efficiency of fecal calprotectin in assessing disease activity of IBD through a meta-analysis.Methods: We searched the following databases: Pub Med, CNKI and Wanfang for studies(with the deadline of December,2014) with fecal calprotectin in evaluating disease activity of IBD. Through reading the articles we collected the data following :turezzz Psgiole–positive(TP),ture –negative(TN),false–positive(FP) and false–negative(FN).Using these data evaluate the weighted sensitivity,specificity,diagnostic odds ratio(DOR) and area under the summary receiver-operating characteristic curve(SROC).Result: There are 17 publications accorded with the standard of inclusion and exclusion.The pooled sensitivity,specificity,diagnostic odds ratio(DOR) and 95% confidence interval and area under the summary receiver-operating characteristic curve(SROC) of FC in evaluating IBD inflammatory activity were 0.842(0.819~0.863),0.806(0.771~0.838),22.137(14.955~32.767)and 0.8934;0.864(0.834~0.891),0.816(0.765~0.859),27.853(16.955~45.757) and 0.911 for UC;0.801(0.762~0.837),0.792(0.735~0.842),14.207(8.031~25.131) and 0.8564 for CD.Conclusion: According to the findings, fecal calprotectin as anon-invasive measurement has a high level of overall accuracy in assessing the inflammatory activity of IBD.
