| Objective To investigate and evaluate the feasibility and clinical effect of bone transportto treat large infected bone defects of lower extremitiesMethods Our study retrospectively analyzed37cases of lower extremities fracture surgeryin patients from January2008to October2013. There were24cases of tibial fractures,13cases offemoral fractures, including27males and11females?average age of34.95±11.35years.Thelength of defects range of was4to12cm?the average defect was7.03±1.95cm. Collect theoperation time, amount of bleeding, fracture healing time, postoperative complications,postoperative Harri?HSS?Baird-Jackson score, and compare the curative effect by the statisticalanalysis.Results37cases were fol lowed up for8to24months, with an average of13.19±3.95months. Resistant Staphylococcus aureus is the most common pathogens of large infectedbone defects. All fractures healing,the average healing time was10.62±2.53months. In theprocess of bone removal,3patients due to the needle pulling the skin to cause pain, suspensiontract ion3to4days until the pain disappears and then continue to move. Postoperative pin tractinfection rate was10.8%,infection control by local dressing and antibiotics. This group ofpatients in3patient postoperative tibial axial off set correct ionthrough the bed to adjust the external fixator.5case got the small contact area of bonelengthening. According to the Paleyevaluation criteria of bone healing and functional recovery after treatment ends, and the recoveryrate of function was96.7%.Then used open micromorselized bone grafting. The function scorehas a significant difference with before(P<0.01).Conclusions Bone transport have an obvious effect in treatment of large infected bonedefects of lower extremities.Using Orthofix external fixor was an effective way of large infectedbone defects,which was indicated by healing time, osteogenesis of the nonunion, and speed ofbone lengthening. |
