ObjectiveOur country is a high-incidence area of gastrointestinal cancer,early detection,early diagnosis and early treatment can significantly improve the survival rate of patients and improve the prognosis and quality of life.Chromoendoscopy is an important method for the diagnosis of early gastrointestinal cancer and can effectively improve the detection rate of the disease.The existing endoscopic gastrointestinal mucosal staining method is to manually configure the dye,and dye the syringe by hand to push the syringe.This process is cumbersome,time-consuming and labor-consuming,and the work efficiency is low.In order to solve the above problems,this study intends to develop a multi-channel intelligent staining pump,so as to optimize the existing endoscopic gastrointestinal mucosal staining,cleaning,water injection and other liquid feeding operations,to provide a new type of equipment for improving the efficiency of medical staff,and exploring a new method to improve the detection of early cancer of the digestive tract.Methods1.According to the principles of smart,efficient,versatile,and easy to use,the design of multi-channel intelligent staining pump includes intelligent control part,liquid driving part,liquid passage part and other fitting parts.The intelligent control part accurately distributes the liquid flow rate and working time.The liquid driving part draws the required reagents to the endoscope.The liquid passage completes the liquid conveying operation.Other accessories canimprove the convenience of operation for liquid feeding under the endoscope.2.According to the actual needs of clinical work,multi-channel intelligent staining pump component selection and parameter setting.According to the principles of program editable,large memory capacity and small chip size,the general-purpose single-chip microcomputer is selected as the core component of intelligent control,and various parameters related to the liquid-feeding operation under the endoscope can be written and programmed;the display is clear and the operation is simple.The principle of choosing the touch-sensitive LCD screen as the operation panel can display the various functions of the multi-channel intelligent staining pump on the screen intuitively,which is convenient for the operator to set and use in the operation;combined with the existing endoscopic liquid feeding operation.The amount of liquid,and based on the principle of adjustable flow rate,stable operation,safety and reliability,the peristaltic pump driven by a stepper motor is selected as a liquid driving device,and the liquid flow provided by the peristaltic pump can meet the use requirements of various endoscopic liquid feeding operations;According to the existing defects and deficiencies in the functional test,the multi-channel smart staining pump was optimized and improved,and the performance test was performed again to verify the feasibility of applying liquid feeding under the endoscope.5.The application of multi-channel intelligent staining pump in esophageal iodine staining: Select 110 patients with suspicious lesions in esophageal tube from gastroscopic examination at the Digestive Endoscopy Center of the General Hospital of Armed Police Forces from June to December 2017 andexclude 110 patients with contraindications.For the experimental group and the control group,55 cases in each group.The experimental group used a multi-channel intelligent staining pump to perform iodine staining on the esophagus,and the control group used a syringe to perform iodine staining on the esophagus.The numbers,time,reagent dosage,lesion detection rate,and medical satisfaction degree of the two groups were recorded.Statistical analysis was performed on the collected data to compare the differences between the two groups and to explore the application value of the multi-channel staining pump in esophageal iodine staining.Results1.Multi-channel intelligent staining pump consists of four parts,namely intelligent control part,liquid drive part,liquid transport part and other parts of accessories.The main microcomputer selected by the multi-channel intelligent staining pump is a general-purpose type,which is smaller than other intelligent chips and has a large memory and can be repeatedly programmed.The touch panel selected in the operation panel has the advantages of low energy consumption,high definition,and simple operation.The intelligent control part selects and sets the intelligent program written in the microcontroller through the operation panel,so that the liquid flow,flow rate,and working time can be precisely controlled,and various functions such as channel switching and pipeline flushing can be realized.The peristaltic pump with a flow rate of0-800ml/min was selected as the liquid infusion device according to the actual endoscopic staining operation.The peristaltic pump was driven by a stepper motor.Compared with the DC motor,it has the advantages of stable performance,safety and reliability,and adjustable output.Can meet the needsof a variety of endoscopic liquid feeding operations.According to the existing endoscopic staining,washing,and drug delivery operations,250 ml and 800 ml reservoir bottles were selected as the reagent storage device.Due to the corrosive and oxidative effects of some reagents used in liquid feeding under endoscopy,medical-grade silicone tubes with strong corrosion resistance and oxidation resistance were pipeline;Imported medical-grade silicone tube lined with Teflon coating can improve the anti-oxidation and anti-corrosion of the pump tube.Service life.The performance test of the optimized and improved multi-channel intelligent staining pump was performed again.The results showed that the multi-channel intelligent staining pump provided the liquid flow rate of 0-800ml/min.The spraying range changed with the flow rate,and the timing could be achieved through an intelligent control program.Quantitative feeding,path switching,pipeline cleaning and other functions can meet the needs of various liquid feeding operations under the endoscope,and the performance is superior to the existing endoscope water pump.The results of the application of multi-channel intelligent staining pump in intestinal mucosal irrigation showed that: the rinse time of the experimental group was 18.01±2.78 s,and that of the control group was 32.96±1.78 s.The t value of the two groups was 10.19,p<0.05.The difference was statistically significant;the water consumption of the experimental group was 56.82±8.91 ml,and the water consumption of the control group was 78.85±15.16 ml.The t value of the two groups was 68.30,p<0.05,and the difference was statistically significant;the mucosal washing of the experimental group was significant.The post-cleanliness score was 2.79±0.91.In the control group,the intestinal cleaning score after the mucosa was washed was 2.30±1.11.The t value of the two groups was 3.043,p<0.05,and the difference was statistically significant.5.The application of multi-channel intelligent staining pump in esophageal iodine staining showed that when a multi-channel intelligent staining pump was used for endoscopic esophageal iodine staining,the process from parametersetting to dyeing operation could be completed by endoscopic surgery.Then the assistants cooperated with the operation to reduce the waste of human resources.The duration of esophageal iodine staining in the experimental group was 15.47±2.72 s,and the esophageal iodine staining time in the control group was 28.17±2.29 s,and the t value was 28.245,p<0.05.The difference between the two groups was statistically significant;the amount of reagent in the experimental group was used.It was 15.84±2.71 ml.The amount of reagent in the control group was 18.73±2.20 ml,and the t value was 6.140,p<0.05.The difference between the two groups was statistically significant.The lesion detection rate in the experimental group was 38.00%.The lesions in the control group were examined.The rate was 26.00% and the x2 value was 1.905,p>0.05.There was no statistical difference between the two groups.The questionnaire showed that the satisfaction of the medical treatment in the experimental group was 90.00%,and the satisfaction of the control group was 58.00%.The x2 value was 6.140,p<0.05,there was a statistical difference between the two groups.Conclusion1.Successfully developed a multi-channel intelligent staining pump that integrates mucosal dyeing,rinsing,water injection,inflating,and drug delivery functions to realize precise control of liquid flow,operation time,channel switching,and tubing cleaning.Can meet the needs of a variety of endoscopic liquid feeding operations.2.The multi-channel intelligent staining pump is safe,reliable,and simple in operation.It reduces the number of operations,the time,and the amount of reagents used for the liquid feeding operation under the endoscope,thereby improving the work efficiency of the medical staff and reducing the work intensity.3.The multi-channel intelligent staining pump has good flushing and dyeing effects on the digestive tract mucosa,improves the detection rate of lesions,and is highly recognized by medical personnel.It provides a new type of equipment for improving the detection rate of early gastrointestinal cancer.
