A Correlation Study Of Adult Human Calcaneal Bone Strength With Body Composition In TianZhu District Of Gansu Province

Objective:Acquire and establish the database of bone mineral density and body composition in Tianzhu, Gansu province by measuring the data of calcaneal bone strength and body composition from the adults of Tibetan and Han to fill the figure's blank of this region. Further understand the change rules of the information in Tianzhu Tibetan region and analyze the difference of bone strength and body composition between different ethnic groups, gender, and age groups, and the database through studying the impact of geographical environment, living habits, heredity and other aspects for the purpose of providing scientific basis for prevention and treatment of the diseases.Method:Using random cluster stratified sampling method, we chose 519 adults from Tibetan Autonomous County of Tianzhu aged between 20 to 90 as research objects, including 184 Tibetans and 335 Hans. We used questionnaire to obtain related information, such as age, level of education, state of economy, drinking, smoking history, dietary habits and exercise. Using quantitative ultrasound bone density, we measured the mineral density of right calcaneal bone to obtain T value, the index of bone strength and the data of fracture risk coefficient. Adopting biological resistance, we measured body composition to obtain the component indicator of BMI, degreasing weight, fat mass, muscles mass, and so on. The test results were grouped according to gender and age. All data were processed by spss19.0 software for statistical analysis.Results:1. In addition to the Tibetan men, the bone strength index decreased along with age in Tianzhu region. The change rule is different between male and female beyond 50 years group, the male group shows slow decrease along with the age, the female group shows rapid decrease with increasing age. The bone strength index of Han Chinese men change more than women; men more than women of Tibetan in just 60 years, others age groups has not statistical significant.(P>0.05).The variation tendency of bone risk opposite with bone mass index.2. The highest rate of detection of Osteoporosis of Tibetan male is the highest in 40-year-old groups, and then it decreases along with the increase of age; while the rate of detection of Osteoporosis is increasing along with the age in the male of Han ethnic group; the female's rate of detection of Osteoporosis of both Han and Tibetan is increasing by the rise of age; the rate of Osseous abnormality of male is lower than female in both Han and Tibetan, but the difference in Tibetan does not have statistical significance.3. The adult body composition of every index does not agree with the increase of the age change trend in Tianzhu region. The body composition index in male Tibetans reached their maximum at their 40-year-old, and then decrease along with the increase of age; as to the male of Han, Mass of body fat and Percent of body fat increase with age, the change rate of other body composition is the same with male Tibetans. In addition to subcutaneous fat, Percent of body fat and Waist-hip ratio increase along with age, other body composition index reached their maximum at their 50-year-old in female Tibetans. The rate of overweight is 34.9%, obesity rate was 11.4%, the calcaneal bone strength overweight and obese group is lower than normal group, and there is no statistically significant difference (P> 0.05).4. The index of bone strength of both Tibetan and Han female, with height, degreasing weight, muscle mass, bone mass, body moisture, intracellular fluid, basal metabolism shows a positive correlation, and negatively correlated with waist-to-hip ratio, and Han female bone strength and protein, body fat rate, visceral fat volume were positively correlated. Han male bone strength index and presumption to fat weight, muscle mass, bone mass, body moisture, fluid in the cell, extracellular fluid, basal metabolism and right lower limb muscle mass are positively correlated, and the intensity of Tibetan male bone index is not associated with the index between body composition.5. Multiple regression analysis shows that only muscle mass of male adult has a positive correlation with bone strength, while the body fat mass, the right lower limb muscle mass of female and bone strength were positively correlated. However, visceral fat mass of male is negatively correlated with bone strength.Conclusions:1. With the increase of age, bone strength index, fracture risk, and body composition show an inconsistent change rule in different ethnic groups and different gender.2. The bone abnormal rate of male is lower than that of female of Han in Tianzhu region, while there is no difference in the rate of bone abnormal between Tibetan male and female.3. The rate of overweight and obesity in Tianzhu region is higher than the national average. however, the rate of obesity seems close to the national level.4. Bone strength of male adult has a positive correlation with muscle mass in Tianzhu, while the body fat mass, the right lower limb muscle mass of female and bone strength were positively correlated. However, visceral fat mass of Tibetan male is negatively correlated with bone strength.
