Research Of The Efficiency Of Government Health Expenditure In Anhui Province

With the advance of medical and health reform, every government continue to increase input in public health and government health expenditure is larger and larger, but the size of the government health expenditure is not unlimited increase,it’s affected by the regional GDP and fiscal revenue, so we increase government health expenditure scale,it’s particularly necessary to pay attention to the efficiency of Medical and health funds.Anhui province has been vigorously practicing the health care reform, during the period of 12th Five-Year, the size of its health care investment has been growing, the health industry has been rapidly developing.2014, Anhui Province, the total health expenditure of 1431.84 million yuan, compared to the eleventh five year, residents of personal expenditures accounted for the ratio decreased, and government expenditure on health accounted for ratio increases, the government undertakes more responsibilities of basic public health services,2011-2014 the growth rate of Anhui province’ average government health expenditure significantly exceeds the growth rate of fiscal expenditure. So with the end of the planning period in 12th Five-Year,it’s practically significant to do the research on the efficiency of government health expenditure in Anhui Province in recent years.Unabridged first introduced the relevant theories of government health expenditure, defines the related concept, and elaborates the theoretical basis of government health expenditure efficiency.The next major analysis the health status of Anhui Province:in absolute size,I compare the growth of health expenditure and fiscal expenditure, on a relative scale, I compare medical health spending accounts for the proportion of GDP and fiscal expenditure changes, along with a brief description of the medical and health resources and services, new rural cooperative medical development.The fourth chapter is the empirical analysis of the efficiency of the government health expenditure analysis in Anhui province for 4 years, and it is also a key point of research.This part firstly introducesthe traditional BBC, CCR model, based on the choice of DEA Malmquist productivity index and the input and output index, then through the traditional DEA model I estimates of static efficiency of the 2011-2014 Anhui provincial government health expenditure, and analysis the differences in technical efficiency, pure technical efficiency, scale efficiency from the dimension of time dimension and area. At the same time, I analysis three intertemporal dynamic efficiency through the Malmquist productivity index in detail and find out the main cause of efficiency change.The study found that 2011-2014 overall efficiency level of Anhui provincial government health expenditure decline,the decline in the average scale efficiency is the main reason for the decline in the overall efficiency of Anhui Province, and most areas did not achieve DEA effective, the area of the efficiency value of the differences are significant, the high input region of the overall efficiency is not the highest, while the lower input area overall efficiency ranking before, health expenditure and efficiency change inversely.Finally, based on the efficiency values of the cities of Anhui province in 2014, mainly from the per capita GDP, population density, urbanization level, the following fiscal decentralization and other economic, social and institutional aspects of the Tobit regression. It was found that the per capita GDP, financial self-sufficiency rate, the proportion of government health expenditure and Anhui provincial government efficiency of health expenditure is negative correlation, urbanization level and positively correlated, and population density, decentralization of fiscal revenue, fiscal expenditure decentralization and government medical and health students spending efficiency is not clear.Therefore, according to the analysis of the above empirical findings and the results of the Tobit regression, this paper puts forward the following improvement of efficiency policy recommendations of Anhui Province health expenditure:first, optimize the Anhui Province health expenditure scale and structure, in the area of increasing returns to scale, increase the health expenditure scale; in the area of decreasing returns to scale,narrow its health expenditure; second, improve budget management, rationally plan the health care resources, promote the of efficiency pure technical; third, improve the fiscal decentralization system, and actively promote the province governing county fiscal system reform, introduct the private capital into the health sector,it is committed to the equalization of health services; fourth, the establish the medical and health local government performance evaluation system, and gradually form output oriented budget concept, adjust the performance evaluation mechanism System, considering the supply of public service into the evaluation system; fifth, accelerate the pace of technological innovation, ensure the continued health care reform.
