With the development of economy and the progress of The Times, social demand for talents is more and more diversified. The old education selects talents by test, and adopts unified standard training of talents, so lead to the phenomenon that some talents is high IQ, but low ability. After the reform and opening up, China ministry of education began to pay attention to the training of all-round development of talent. 2001, our country foundation education implements the new course standard, pays attention to the training and development of the talents’ innovative and practical ability, turns the teachers’ leading position to the student’s main body status, and pays attention to students’ personality development in the open teaching. To student’s evaluation, turn the old way of summative evaluation, to pay more attention to the students’ process development.In this paper, according to the curriculum reform, the student’s evaluation is changed, and combines with some scholars research on learning evaluation, D-H evaluation model is proposed. This article uses the method of literature review, case method, questionnaire and observation method, and researchs D-H evaluation model from the theoretical basis, model building, model application and evaluation. D-H evaluation is based on constructivism and the theory of distributed cognition, and is a diverse and hierarchical evaluation witch combines with curriculum standard. In this study, a total of five evaluation main body, including the teacher in charge, the deputy head teacher, self-evaluation, group members, and parents. They evaluate students from different angles, the feedback evaluation of student learning development seems to be more close to the actual development level of students. Evaluation level according to the evaluation content and the curriculum standard is divided into three levels, the evaluation main body according to the actual performance of the evaluators gives corresponding grade assessment, the assessment of different subject has different content, but the same evaluation content may exist between different evaluation main body interaction phenomenon.D-H evaluation model, for students’ learning condition can be omni-directional and multi-level evaluation, the evaluation of pay attention to the students’ learning process evaluation, but also pay attention to the students’ learning result evaluation. D-H learning evaluation results, not to give students a comprehensive ranking, but the feedback of students’ learning, personalized teaching for teachers according to the different situation of the students. Purpose is according to different students’ learning situation, proposes the cultivation of the development direction, promote the student individual ability of longitudinal development.
