Nowadays, the population aging in China is getting more serious than ever. The amount of people who’s older than 60 is more than 220 million. Disability, aging, the tendency of small-scale families, old people living separately with their adult children and the speed of the flow of people, which all make how to take care of old people a problem. Aging before getting rich and the aging security system is incomplete, which put rural area in a much more difficult situation. However, medical treatment cannot be separated from take care of old people. The State Council issued several document about the cooperation of health care department and pension service department. Literature research, questionnaire survey, as well as interview have been used for exploring the factors associated with preferences for institutional care of the elderly in rural China. Based on the results from the exploring, recommendations and suggestions have been put forward to improve the investment on public resources and the construction of the pension institutional.The study shows that there are eight factors which have statistical differences on the rural area elder’s living willingness of pension institutional, including the age, education, annual income, Activities of Daily Living (ADL), emotional support, the access to medical resource, how many chronic diseases do elder have, and the attitude towards bring up their children for old age. This paper also adopted the Logistic Regression Model to explore the factors affected the rural area elder’s living willingness of pension institutional and on what degree can they affected.Form the exploring, research status at home and abroad, this paper propose that we have to improve the construction of the pension institutional based on the following things:the needs of the elder; provide available medical resources; pay attention to emotional support and psychological counseling; attach importance to old and low income people; set up a multi-level regional pension system.
