Analysis Of Status, Causes And Countermeasures For Medical Dispute Of Changsha City During2008-2011

Objective:The distribution、dispose and compensation of medical dispute of Changsha city during2008-2011was described,and the impact factors of medical disputes in hospitalized patients was analyzed. According to the results of analysis, discuss the preventive measures of medical malpractice.Methods:Descriptive analysis was used for the distribution、dispose and compensation of medical malpractice. Medical records of532normal patients from six hospitals were extracted by Randomization, and to compare with the disputes patients. Univariate and multivariate analysis was used for the impact factors that may affect medical disputes.Results:The average rate of medical disputes was0.0149%。 between2008-2011. The number of medical disputes of tertiary hospitals is higher than secondary hospitals.surgical department、outpatient and emergency department gynecology is high-risk department respectively. During study period,85.332%medical dispute was handled by negotiation, the total amount of compensation increased year by year. Univariate analysis showed that age, occupation, admission illness, the time of disease diagnosis, duration of hospitalization, disease outcome, consultation frequency, communication frequency between doctor and patient, medical expenses were the impact factors. Multivariate analysis showed that the disease outcome, frequency of consultation, medical fees were independent risk factors.Conclusions:The incidence rate of medical disputes remains stable."Disease outcome, frequency of consultation, medical fees" were primary risk factors. To prevent medical disputes must consider the following four aspects:"Enhance the monitoring of the medical risk, establish and perfect early warning mechanism for medical dispute, further to construct a harmonious doctor-patient relationship, realize the diversity of resolution mechanism for medical dispute".
