Objective To exlpore high-risk sexual behavior and associated factors though trajectory model among men who have sex with men(MSM).To explore whether exist sexual risk compensation in the HIV pre-exposure prophylaxis clinical trial(Pr EP), so as to provide reference for the future study of Pr EP among MSM.Method A clinical trial of oral Pr EP to prevent HIV infection among MSM was conducted with non-probabilistic sampling used for subjects recruitment n Chongqing, Sichuan, Xinjiang and Guangxi. After the baseline questionnaire survey, the eligible subjects exclusion criteria were randomized grouping and assigned drugs for the prevention of HIV. Clinical follow-up and questionnaire were carried at 12 th,24th,36 th,48th week.(1)MSM were divided into different subgroups through trajectory model and describe the high-risk sexual behavior of different groups. Univariate and multivariate regression were used to analyze associated factors.(2)Analysis of repeated measures were used to analyze the sexual risk compensation in the Pr EP among MSM.Results(1)Totally 534 MSM were included in the cohort study. The subjects can be divided into high risk group(199 MSM) and low risk group(335 MSM) through trajectory model and high-risk sexual behavior times were 1.16 and 0.50 times in last two weeks, respectively. Multivariate analysis showed that marriage, income, HIV consulting and medication compliance had statistical significance(p<0.05).The Low-income(OR=1.86,95%CI=1.28~2.71),did not do HIV consulting(OR=1.65,95%CI=1.09~2.50)participants and divorced/ widowed(OR=2.18,95%CI=1.02~4.76)had a higher chance of entering the high risk group. Compared with “blank controlâ€, "high compliance" and "low compliance"(OR=1.77,95%CI=1.15~2.72;OR=1.71,95% CI=1.05~2.81)had a higher chance of entering the high risk group.(2)There were known some information about the sexual risk compensation in the Pr EP clinical trail among MSM by analysis of repeated measures. In the number of sexual partners, the changes were not statistically significant in the baseline survey and follow-up survey between the three groups of "high compliance", "low compliance", and "blank control"(P>0.05). In the high-risk sexual behaviors, the changes were not statistically significant in the baseline survey(P>0.05)and there were statistically significant in the follow-up survey(P<0.05). Average high-risk sexual behaviors of "high compliance" 0.24 times higher than "blank control".Conclusion(1)The trajectory analysis model can directly reflect the different development paths of MSM high-risk sexual behavior. The low-income participants and divorced/widowed should be focused. We need to strengthen the publicity of HIV related knowledge.(2)There were some evidence about high-risk sexual behavior of the sexual risk compensation in the Pr EP clinical trail among MSM. We should place great emphasis on sexual risk compensation in the Pr EP clinical trail. |
