| Objective: To investigate the hepatitis B surface antibody(HBs Ab) level after hepatitis B vaccine(Hep B) in Chongqing, and analysis the immune effect of Hep B in different age groups child and its influencing factors. To evaluate persistent immune effect of Hep B, and to examine the feasibility of booster program of Hep B, So as to provide scientific basis for improving of Hep B immune program for children in Chongqing.Method: A total of 28620 children were selected according to the exclusion criterion hepatitis B patients from Children’s hospital of Chongqing Medical University in 2014.Collect all the HBV serological makers test results, history of vaccination, and source of data for statistical analysis.HBVM and HBs Ab level were detected by CMIA. According to the age,all the members can be divided into 7 groups, 0~28 days, 1~12 months group, 1~3 years old, 4~6 years old, 7~10 years old, 11~14 years old and 15~18 years old. To all the datas, using SPSS21.0 statistical software for statistical analysis.Results:1. Hepatitis B vaccine inoculation:Among the 28620 childrens, full course vaccination rate is 97.93%, and the vaccination rate is significantly different between the different age groups.2. HBV serological makers test results:The positive rates of HBs Ag, anti-HBs, HBe Ag, anti-HBe and anti-HBc were 0.59%, 68.82%, 0.23%, 2.86% and 1.35%. HBs Ab positive rate increased after the birth, reach the highest of 92.4% in 1~years old,and then showed a trend of decline gradually, fall to the lowest 44.2% in the 7~8 years old grou, totally falling 48.2%; But there were recovered again in 11~15years old group, and continued again small amplitude. Significant difference exists among different age groups with HBs Ab positive rate. HBs Ab positive rate and level is gradually declining as the growth of the age.3. Hepatitis B vaccine immune response : The geometric mean concentration of HBs Ab is 31.96 m IU/m L, and its level is mainly influenced by age and inoculation duration(P<0.05). The GMC up to 141.66 m IU/m L in 1 years old group, followed by 2years old, and after 3years old the GMC gradually decling, and down to the lowest level in 7~8 years old of 9.32~11.93 m IU/m L. Immune response was 68.82% after Hepatitis B vaccine immunization(HBs Ab≥10 m IU/m L). Different interval time, the immune response to maintain(low, medium and high response) difference were significant(P<0.01). Conclusions:1. Children hepatitis B vaccination rates in Chongqing is higher, shows that Chongqing area hepatitis B vaccine immunization work effect is remarkable.2. Hepaitis B vaccine immunization effect and durability was attenuaton as the growth of age and the extension of interval time, The HBs Ab GMC was at a lower level at 7~8years old, lower than the critical value of 10 m IU/m L. So it is suggested that we should strength screening the immunization effect in school-age children, and hepatitis B vaccine immunization can be strengthened for this group children when it is necessary. |
