| Diabetic radiculoplexus neuropathy(DRPN) is a rare type of diabetic neuropathy, and characterized by pain and weakness, which is accompanied by dysfunction of autonomic nervous system. DRPN begins unilaterally and focally,but can spread to be multifocal and bilateral, even involving innervation area in the distance. It often includes three kinds of anatomical types:diabetic lumbosacral radiculoplexus neuropathy(DLRPN), diabetic thoracic radiculoneuropathy(DTRN), diabetic cervical radiculoplexus neuropathy(DCRPN). DLRPN is the most common type of DRPN,onset alone or in combination with other types. DRPN has complex pathogenesis,the main pathogenesis is immune-mediated microvasculitis, resulting in ischemic injury to the nerves;metabolic derangement may be also a risk factor. Most patients can be self-healing, but few patients still leave weakness and pain, the incidence of disability is higher than expected.Immune therapy, controlling pain and other comprehensive treatment can improve the quality of life. In this paper, the potential pathogenesis, clinical characteristics and therapies of DRPN will be reviewed in detail. |
