Objective To analysis the short-term effectiveness after static functional reconstruction of the Fanelli C type of posterolateral complex(PLC) by the anterior half of autologous peroneus longus tendon.Methods A total of 32 cases with the injury of PLC were treated in our hospital from June 2012 to November 2014.CPM was used to promote recover of the knee joint activity of patients with knee joint stiffness caused by obsolete injuries. Then the knee joint stability was examined and all individuals had varus and exteral rotation instability.According to Fanelli typing,all cases were classified typically as type C.ACL and/or PCL were reconstructed simultaneously with autologous ipsilateral semitendinous and gracilis tendon(if necessary,taking bilateral),and PLC was reconstructed with the anterior part of autologous ipsilateral peroneus longus tendon.Knee joint activity, the lateral compartment opening on varus stress radiography at 0°and 30°of flexion,the dial test at 30°and 90°of the knee flexion,the Lysholm score and IKDC score of the knee were compared to assess the effect of the surgery preoperatively and postoperatively.Results In all cases,the follow-up period was 12 months.At the final follow-up evaluation.the knee joint activity(preoperative,74.75°±11.56° vs postoperative,106.44°±9.75°;p=0.00),the lateral compartment opening on the varus stress radiography(at 0°:preoperative,14.17 mm ± 1.37 mm vs postoperative,4.25 mm ± 0.84mm;p=0.00; at 30°:preoperative,14.59mm±3.05 mm vs postoperative,4.22mm±0.94mm;p=0.00),the dial test(at30°:preoperative,13.95 °± 1.70° vs postoperative,5.50 °± 1.56; p=0.00;at 90°: preoperative,14.64°±1.90°vs postoperative,5.86°±1.51°;p=0.00), Lyshlom knee score(preoperative,47.09 ± 8.88 vs postoperative,82.09 ± 6.18;p=0.00). According to IKDC scoring, all patients were rated as grade D before operation,but 10 cases were rated as grade A, 18 as grade B, and 4 as grade C after operation(Z=5.36, P=0.00).The results above were statistically significant.There were 3 cases superficial infection,2 cases with knee joint stiffness and 3 patients with lateral II degree relaxation under the stress of the inversion.Conclusion The reconstruction for the Fanelli C type of the posterolateral corner injury by using the anterior part of autologous peroneus longus tendon can obtain satisfactory short-term clinical outcome and effectively reconstruct the restoration stability of posterolateral of knee.
