Objective: To study the correlation of Brain Natriuretic Peptide and the Severity of Coronary Artery Disease in the patients with both Hypertension and Coronary Heart Disease.Methods: 90 patients with Hypertension were enrolled in this study. All recruited patients underwent Coronary Angiographic Examination. According to CAG result, they were classified into 2 groups: Hypertension group(with negative CAG result,n=45) and Hypertension and Coronary Heart Disease group(with positive CAG result,n=45). According to clinical manifestation, electrocardiographs and the levels of biochemical markers of myocardial necrosis, Hypertension and Coronary Heart Disease group were classified into 3 groups: Stable Angina group(SA,n=16)Unstable Angina group(UA,n=14)and Acute Myocardial Infarction group(AMI,n=15).Hypertension and Coronary Heart Disease group was divided into single- vessel(n=16),double-vessel(n=14)and triple-vessel group(n=15).The single- vessel group is further divided into anterior descending artery lesion group(n=8), circumflex artery lesion group(n=5) and the right coronary artery disease group(n=3). Coronary angiography from the elbow vein blood samples before analysis of the enzyme- linked immunosorbent assay method for the determination of plasma levels of BNP. 45 subjects who underwent positive CAG result and calculated Gensini score.The BNP levels were compared two groups,and analysised with the conorary lesion vessel and Gensini score. Result: 1. In Hypertension and Coronary Heart Disease group,the level of BNP significantly higher in AMI groups than in other three groups(385.23±83.16pg/ml vs 205.49± 68.92 pg/ml,385.23±83.16pg/ml vs 77.35±18.83 pg/ml, 385.23±83.16pg/ml vs 20.58±5.24 pg/ml P<0.05),and was marked higher in UA than in SA groups(205.49±68.92 pg/ml vs 77.35±18.83 pg/ml,P<0.05),also was higher in SA than in Hypertension groups(77.35±18.83 pg/ml vs 20.58±5.24 pg/ml,P<0.05).2.In Hypertension and Coronary Heart Disease group,the level of plasma BNP among the lesions of 1,2,3 vessels were293.01±77.07 pg/ml vs 124.35±47.36 pg/ml,124.35±47.36 pg/ml vs 70.40±18.76 pg/ml P<0.05.3. In single- vessel group,plasma BNP concentrations in patients with left anterior descending artery was higher than those of patients with right coronary artery and left circumflex coronary artery(149.87±40.00pg/ml vs 54.87±15.17pg/ml,P<0.05, 149.87±40.00pg/ml vs33.08±11.27pg/ml,P<0.05), the circumflex artery and right coronary artery lesion group BNP level group difference was not statistically significant(54.87±15.17pg/ml vs 33.08±11.27pg/ml,P>0.05).4. With the comparison and analysis the level of BNP and Score(r=0.842,P<0.01), we found the level of BNP tended to increase with the aggravation of coronary artery stenosis. TIMI flow showed a sig nalficant negative correlation with plasma BNP levels( rs=-0.633, P<0.01).Conclusion:1.Plasma BNP significantly increased in patients with with both Hypertension and Coronary Heart Disease,prompting level of plasma can be used to predict the severity of myocardial ischemia.2. Support the greater the number of coronary artery lesion, vascular the more important crimes, the higher the level of plasma BNP,prompting level of plasma BNP can be used to predict the severity of Coronary Artery Disease.3.The level of plasma BNP and Gensini score is positive, and TIMI bleeding score is negative, BNP level was positively correlated with the degree of coronary stenosis.
