Objective:Recent research indicates that the intervertebral disc cell senescence and the inflammatory factor (IL-1 beta) secreted by aging cells are closely related to intervertebral disc degeneration. To compare the expression levels between the normal group and degeneration group, the expression of p53 and interleukin-lbeta (IL-1 beta), interleukin-6 (IL-6) were detected in normal and degeneration nucleus pulposus of lumbar intervertebral discs. To investigate the contribution and significance of p53, IL-1β, IL-6 in intervertebral disc degeneration, by analyzing the correlations among p53, IL-1β, IL-6,and the correlations between intervertebral disc degeneration grade and the expression of p53, IL-1β, IL-6.Methods:The tissues of the degenerated NP were obtained from 40 Lumbar disc herniation (LDH) patients (22males,18 females), the herniation part:L4/5.These tissues have included in normal group after been confirmed by postoperative pathology. As the control group, normal lumbar discs were obtained from 20 Lumber Vertebrae Fracture patients (14males,6 females), the herniation part:L4/5, confirmed by postoperative pathology. The nucleus is divided into five levels according to Thompson classification. The expression of p53,IL-1β and IL-6 was quantified by immunohistochemical staining method. Those above were classified separately by cell staining positive rate. The cell staining positive rate of p53, IL-1β and IL-6 between normal and degeneration group were compared with chi-square test. At the same time, the correlation of disc degeneration grade according to Thompson score with the expression of p53,IL-1β and IL-6 were analyzed with spearman test. To detect the expression of p53,IL-1β and IL-6 in nucleus pulposus specimens by Western-Blot. The two groups were compared with paired t test. At the same time, the correlation of expression of p53 and IL-1β, IL-6 were analyzed by spearman test.Results:1. The results of immunohistochemical staining:Positive staining rate of p53,IL-1β and IL-6 in degenerated NP were markedly higher than that in control NP(80%vs.10%,87.5% vs.20% and 72.5% vs.15% respectively, both P<0.001).A positive correlation was observed between the degeneration grade and the expression of p53,IL-1β and IL-6 (rs=0.616,rs=0.673 and rs=0.654,respectively, p<0.001).2. Western Blot results showed that the expression of p53 and IL-1β, IL-6 in degeneration group were significantly increased compared with normal group. There was statistically significant difference(P<0.05) of gray level ratio in two groups of p53 and IL-1β, IL-6. The expression of p53 was positively related respectively with the expression of IL-1β and IL-6. The correlation coefficient of p53 and IL-1β:rs= 0.466, p=0.003,moderate correlation; The correlation coefficient of p53 and IL-6:rs=0.447, p=0.004,moderate correlation. There was no correlation between the expression of IL-1β and IL-6.(rs=0.113,p=0.512).Conclusion:1. The higher expression of p53, IL-1β, IL-6 in the degenerated NP indicated that p53, IL-1β, IL-6 were closely associated with the degenerative process of intervertebral disc (IVD).2. The positive correlation between the expression of p53, IL-1β, IL-6 and degeneration grade indicated that p53, IL-1β, IL-6 are important factors for intervertebral disc (IVD).3. The positive correlation of the expression between p53 and IL-1β, IL-6 indicated that p53 play an important role in the intervertebral disc degeneration maybe associated with IL-1β and IL-6.
