Objective:Periodontitis which is the dental plaque infect gingival and periodontal tissue induced a chronic inflammation.It is a destructive disease which is c haracterized by the formation and inflammation of periodontal pocket,absorption of alveolar bones and tooth mobility. It is periodontitis that lead to adult tooth loss. Obstructive sleep apnea- hypopnea syndrome(OSAHS) is a systemic disease.Its main pathophysiological basis is oxidative stress which is lead by intermittent hypoxia and enhance of inflammatory response, and then lead to multiple organs damage.Research has shown that the incidence rate of OSAHS patience with periodontitis is obviously higher than normal people. In order to explore the relationship between OSAHS and periodontitis,we simulate the same condition of intermittent hypoxia of OSAHS,and modeling periodontal rats. To observe the change of rat’s periodontal tissue, and detect the expression of CRP and leptin in rat’s gingival tissue and serum.Then investigate the two indexes’ s destroy mechanism of rat’s periodontitis in chronic intermittent hypoxia. Methods:1.Modeling periodontal rats in chronic intermittent hypoxia, we chose 32 healthy male rats which weights 200 g, and then divided 8 rats into each four groups randomly. Normoxia group, periodontitis group, hypoxia group, hypoxia periodontitis group(composite group). The periodontal model was established by ligated the second molar of maxillary and raised with periodontal diets. Normoxia group and periodontitis group were raised in the condition of normoxia environment, while hypoxia group and composite group were raised in the hypoxia environment separately. Executed all rats after 2 months.2. We detected the clinical indicators of periodontal in each group after 8 weeks.3. We draw blood from abdominal aorta, at the same time take the heart,lung, kidney and gingival tissue of experimental teeth, then kill the rats and dissect maxillary bone for X-ray shooting in order to observe the loss of alveolar bone.4. We use the ELISA to detect the change of leptin and CRP in blood serum and gingival tissue. Results:1. The inflammation of composite group was most obviously. We can probe a deeper periodontal pockets, the plaque index(PLI), bleeding index(BI) and attachment lost(AL) were significantly higher than other groups(P<0.05).2. The pathological changes of heart, lung and kidney were observed under light microscope after HE staining after 2 months. The rats in hypoxia group and composite group were found for myocardial cell swelling and cell cytoplasm was unevenly distributed, which suggested the myocardial remodeling in the heart. Hyaline degeneration and thickening arterial wall were founded in hypoxia group and composite group and also thickening p ulmonary arteriolar wall thickening and stenosis can be observed in this two groups.3. We observed the X-ray of all the experimental teeth after 8 weeks. The alveolar bone height of periodontitis group and composite group was obviously decreased than the other two groups, and the loss of alveolar bone height was more serious in composite group, which explanation the rats model were successed at about eight weeks.4.Leptin and C RP concentration were statistical differences among the four groups(P<0.05), and the leptin concentration in composite group was significa ntly higher than the other three groups, the CRP concentration in gingival tissue was increase whereas the leptin concentration was decreased. Conclusion:1. This experiment simulating intermittent hypoxia environment in low oxygen tank, and caused material injury to important organ of rats.2. We successfully established the rats periodontitis model under simulated OSAHS condition. The periodontal tissue’s inflammation was obviously and the absorption of alveolar bones was seriously.3. In chronic intermittent hypoxic environment, the level of leptin and CRP in blood serum increased but the level of leptin in gingival tissue decreased, suggesting that intermittent hypoxic may lead to periodontitis and the incidence of periodontitis was possibly related to the change of leptin and CRP.
